摘  要:目的回顾性分析2020年唐山市妇幼医院儿童肺炎链球菌(Sp)耐药现状,研究Sp多重耐药菌株中dpr基因突变情况及对抗生素耐药和生化反应产生的影响,为本地区Sp感染的预防和临床用药提供依据。方法收集2020年1~12月医院患儿Sp菌株,根据菌落形态以及全自动细菌鉴定/药敏检测系统,统计儿童Sp耐药率;随机抽取42例Sp菌株进行dpr基因测序,将其测序结果与genebank中SpR6株的dpr基因进行blast比对;统计分析基因突变情况及其与抗生素耐药的关系、对生化反应的影响。结果本次研究结果显示Sp的分离率为26.9%,居2020年儿童感染性疾病首位。男女比例为1.6:1;四季均有发病,以秋、冬季发病明显增多;年龄12~36个月的儿童感染率最高;耐药统计中未发现对万古霉素、利奈唑胺的耐药株;奎奴普丁、红霉素、克林霉素、四环素的耐药率较高,分别为99.9%、98.8%、98.4%、94.8%。本研究发现多重耐药的Sp其dpr基因突变率为40.5%,且dpr基因突变与复方新诺明耐药性相关(P<0.05),与腺嘧啶脱氧核苷(THY)生化反应也相关(P<0.05)。结论肺炎链球菌多重耐药菌株dpr基因突变率较高,并且基因的变异与特定的抗生素耐药性相关,影响某些生化反应结果。Objective To retrospectively analyze the current drug resistance status of Streptococcus pneumoniae(Sp)in children at Tangshan Maternal and Child Hospital in 2020,study the mutation of the dpr gene in Sp multidrug-resistant strains and its impact on antibiotic resistance and biochemical reactions,and provide basis for the prevention and clinical medication of Sp infection in this area.Methods Collecting Sp strains from hospital children from January to December 2020,and calculating the Sp resistance rate of children based on colony morphology and fully automated bacterial identification or drug sensitivity testing system;Randomly selecting 42 Sp strains for dpr gene sequencing,and comparing their sequencing results with the dpr genes of SpR6 strains in the gene bank for blast comparison;Statistical analysis of gene mutations and their relationship with antibiotic resistance,as well as it impacting on biochemical reactions.Results The results of this study showed that the isolation rate of Sp was 26.9%,ranking first in children's infectious diseases in 2020.The male to female ratio is 1.6:1;The disease occurs throughout the four seasons,with a significant increase in autumn and winter;Children aged 12-36 months have the highest infection rate;No resistant strains to vancomycin or linezolid were found in the drug resistance statistics;The drug resistance rates of quinuputin,erythromycin,clindamycin,and tetracycline are relatively high,with 99.9%,98.8%,98.4%,and 94.8%,respectively.This study found that the mutation rate of the dpr gene in multidrug-resistant Sp was 40.5%,and the mutation of the dpr gene was associated with the drug resistance of compound sulfamethoxazole P<0.05,as well as with the biochemical reaction of adenosine deoxyribonucleotide(THY)P<0.05.Conclusion The mutation rate of DPR gene in multi resistant strains of Streptococcus pneumoniae is relatively high,and the gene mutation is related to specific antibiotic resistance,which affects certain biochemical reaction results.
