免疫荧光染色在真菌性角膜炎刮片标本中的诊断价值

摘　　要：目的探讨免疫荧光染色技术在真菌性角膜炎诊断中的价值。方法选择我院眼科就诊疑似真菌感染患者的角膜刮片标本,分别运用免疫荧光染色一步法、PAS染色法、KOH湿片法对这些标本进行检测,记录数据;以真菌培养结果为金标准,比较三种真菌检测方法的准确率、灵敏度、特异度、阳性预测值和阴性预测值。结果与KOH湿片法、PAS染色法真菌检测结果对比,免疫荧光染色真菌检测的准确率、灵敏度、特异度、阳性预测值和阴性预测值较高,差异均具有统计学意义(P<0.05)。结论在角膜真菌感染的诊断中,免疫荧光染色真菌检测的准确率、灵敏度、特异度、阳性预测值和阴性预测值均高于KOH湿片法和PAS染色法,而且此方法检测速度快,易于观察。Objective To explore the value of immunofluorescence staining in the diagnosis of fungal keratitis.Methods The corneal scrape specimens with suspected fungal infection in the ophthalmic outpatient department of our hospital were selected.The specimens were detected by one-step immunofluorescence staining,PAS stainingand KOH wet film methods,and the data were recorded.Using the fungal culture results as the gold standard,the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of the three detection methods were compared.Results Among the three detection methods,the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of immunofluorescence staining for fungal detection were highest compared with KOH wet film method and PAS staining method(P<0.05).Conclusion In the diagnosis of corneal fungal infection,The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of immunofluorescence staining for fungal detection are highest than those of KOH wet film method and PAS staining method.Moreover,this method is fast and easy to observe.
