“剂量效应”对孕产妇不规则抗体特异性鉴定的影响

摘　　要：目的观察不规则抗体鉴定过程中剂量效应出现的条件,探讨剂量效应对孕产妇不规则抗体特异性鉴定的影响。方法通过微柱凝胶法对抗体筛查结果为阳性的血浆标本进行不规则抗体特异性鉴定。针对Rh、MNS、Lewis、Duffy、Kidd血型系统抗体阳性的标本进行倍比稀释并分别与纯合子及杂合子谱细胞反应,观察纯合子和杂合子抗原阳性谱细胞反应的凝集强度的差异。采用Bonferroni法比较杂合子细胞对孕产妇不规则抗体的漏检情况。结果待检血浆与杂合子谱细胞反应为阴性时的抗体效价倍数低于与纯合子谱细胞反应的抗体效价比较倍数,出现剂量效应。当待检血浆与纯合子谱细胞反应凝集强度<2+时,杂合子谱细胞对不规则抗体的漏检率增加(P<0.05)。结论待检血浆中的抗体效价过低及杂合子谱细胞的单剂量抗原,均可能干扰孕产妇Rh、MNS、Duffy、Kidd血型系统不规则抗体的特异性鉴定。Objective To observe the conditions of dose effect in the process of irregular antibody identification,and to explore the effect of dose effect on the specificity identification of irregular antibody in pregnant women.Methods The plasma samples with positive results of antibody screening were identified for irregular antibody specificity by microcolumn gel method.The samples with positive antibodies of Rh,MNS,Lewis,Duffy and Kidd blood were diluted and reacted with homozygous and heterozygous cells respectively,and the difference of agglutination intensity between homozygous and heterozygous antigen positive cells was observed.The missed detection of irregular antibodies in pregnant women by heterozygous cells was compared by Bonferroni method.Results The antibody titer when reaction of plasma with heterozygote spectral cells was negative was lower than that with the homozygote spectral cells,and the dose effect appeared.When the agglutination intensity between plasma and homozygous panelcells was less than 2+,the missed detection rate of irregular antibodies in heterozygous spectral cells increased.Conclusion The low titer of antibody in plasma and the single dose antigen of heterozygous cells may interfere with the specificity identification of irregular antibodies in Rh,MNS,Duffy and Kidd blood group system of pregnant woman.
