银翘清咽颗粒治疗小儿呼吸道合胞病毒急性支气管炎的疗效观察

摘　　要：目的研究银翘清咽颗粒治疗小儿呼吸道合胞病毒(RSV)急性支气管炎的疗效与作用。方法选取2021年1月~2021年12月广东省第二中医院儿科及发热门诊RSV急性支气管炎患者86例,随机数字生成器分为对照组与观察组,每组43例。对照组予盐酸丙卡特罗口服液与布洛芬对症治疗,观察组予银翘清咽颗粒治疗,两组均连续治疗5d。观察两组患儿治疗前后症状和体征,观察两组患者退热所需时间、咳嗽消失所需时间,采用中医证候评分进行总体疗效评价。酶联免疫法检测两组患儿治疗前后血清白介素(IL)-6、巨噬细胞迁移抑制因子(MIF)的含量。观察两组患者RSV转阴率与不良反应发生情况。结果观察组总有效率为95.35%,高于对照组的79.07%,差异有统计学意义(χ^(2)=5.11,P=0.02<0.05)。观察组显效率为72.09%,高于对照组的48.84%,差异有统计学意义(χ^(2)=4.86,P=0.03<0.05)。观察组治疗后中医证候评分低于对照组,差异有统计学意义(P<0.05)。观察组退热所需时间与咳嗽消失所需时间均少于对照组,差异有统计学意义(P<0.05)。观察组治疗后血清IL-6与MIF含量低于对照组,差异有统计学意义(P<0.05)。观察组RSV转阴率为93.02%(40/43),高于对照组的65.12%(28/43),差异有统计学意义(χ^(2)=10.12,P=0.00<0.05)。两组患者治疗期间均未见不良反应发生。结论银翘清咽颗粒可有效改善RSV感染急性支气管炎患儿的临床症状,其作用与抑制IL-6与MIF水平有关。Objective To investigate the effect of Yinqiaoqingyan Particles in treating the pediatric acute bronchitis infected by respiratory syncytial virus(RSV).Methods 86 patients with RSV acute bronchitis were divided into control and observation group by random number generator,and there were 43 patients in both groups.The control group was symptomatically treated with procaterol hydrochloride oral solution and ibuprofen,while the observation group was treated with Yinqiaoqingyan Particles.All patients were treated with 5 days.The symptoms and signs were observed before and after the medication,and the time needed to relive the fever and cough were observed.The TCM syndrome scores were used to evaluate the total effect.The serum concentra-tions of interleukin(IL)-6 and macrophage migration inhibitory factor(MIF)before and after the treatment were tested by en-zyme-linked immunosorbent assay.The negative conversion ratio of RSV and the adverse reaction recurrence were observed.Re-sults The total effective rate of observation group was 95.35%,higher than the control group(79.07%),and there was the difference(χ^(2)=5.11,P=0.02<0.05).And the remarkable effective rate of observation group was 72.09%,higher than the control group(48.84%),and there was the difference(χ^(2)=4.86,P=0.03<0.05).After medication,the TCM syndrome scores of observation group was lower than the control group with difference(P<0.05).The time needed to relieve the fever and cough in observation group was less than the control group with difference(P<0.05).Indeed,the serum concentrations of IL-6 and MIF in observation group after treatment were obviously lower than the control group and there were the differences(P<0.05).Moreover,the negative conversion ratio of RSV in observation group was 93.02%,higher than the control group(65.12%),and there was the difference(P<0.05).There was no adverse reaction in both groups.Conclusion Yinqiao-qingyan Particles could improve the manifestations of pediatric patients with acute bronchitis infected by RSV,and the
