血清BNP、ANCA、CRP、ESR在小儿不典型川崎病中的水平变化及诊断价值

摘　　要：目的探讨血清脑钠肽(BNP)、抗中性粒细胞抗体(ANCA)、C-反应蛋白(CRP)、红细胞沉降率(ESR)在小儿不典型川崎病(IKD)中的水平变化及诊断价值。方法此次研究对象均来自粤北人民医院,选取时间为2021年2月-2023年2月,采用前瞻性随机对照研究,将符合IKD诊断标准的30例患儿作为观察组,将30例川崎病患儿作为对照组,对两组患儿的血清BNP、ANCA、CRP、ESR水平进行测定并比较,比较两组患儿血常规相关指标,以影像学诊断结果为金标准,分析观察组血清BNP、ANCA、CRP、ESR水平的诊断价值。结果与对照组相比,观察组血清WBC、PLT、LC水平及血清BNP、ANCA、CRP、ESR水平均更高,Hct、Hemoglobin水平均更低(P<0.05)。在30例IKD患儿中,与CRP、ESR相比,ANCA、BNP的灵敏度、特异度、准确率、阳性预测值、阴性预测值较高。结论在IKD患儿中,与CRP、ESR相比,ANCA、BNP的整体诊断价值较高,可作为临床对早期IKD患儿进行诊断的检测指标,为早期治疗IKD患儿,降低其并发症发生风险及提升患儿生存率提供科学依据。Objective To investigate the changes and diagnostic value of serum brain natriuretic peptide(BNP),anti neutrophil antibody(ANCA),C-reactive protein(CRP),and erythrocyte sedimentation rate(ESR)in children with incomplete Kawasaki disease(IKD).Method The subjects of this study were allfrom the People's Hospital of North Guangdong,from February 2021 to February 2023.A prospective randomized controlled study was conducted,with 30 children who met the IKD diagnostic criteria as the observation group and 30 children with Kawasaki disease as the control group.The serum BNP,ANCA,CRP,and ESR levels of the two groups of children were measured and compared.The blood routine related indicators of the two groups of children were compared,and the imaging diagnostic results were used as the gold standard,Analyze the diagnostic value of serum BNP,ANCA,CRP,and ESR levels in the observation group.Results Compared with the control group,the observation group had higher levels of serum WBC,PLT,LC,BNP,ANCA,CRP,ESR,and lower levels of Hct and Hemoglobin(P<0.05).Among 30 children with IKD,ANCA and BNP showed higher sensitivity,specificity,accuracy,positive predictive value,and negative predictive value compared to CRP and ESR.Conclusion Compared with CRP and ESR,ANCA and BNP have higher overall diagnostic value in children with IKD,and can serve as diagnostic indicators for early IKD in clinical practice.They provide scientific basis for early treatment of IKD,reducing the risk of complications,and improving the survival rate of children.
