超声引导下外周神经阻滞麻醉在孕妇下肢骨折手术中的临床应用及疗效观察

摘　　要：目的观察超声引导下外周神经阻滞麻醉在孕妇下肢骨折手术中的临床疗效及安全性。方法此研究选取2020年1月-2022年10月在白银市中西医结合医院骨伤科手术治疗的下肢骨折的20例孕妇作为观察组,主要为髋骨骨折及外踝骨折,采用超声引导下神经阻滞麻醉进行手术;同时随机选取骨及外踝骨折的非受孕患者30例作为对照组,采用椎管内麻醉进行手术;观察两组患者麻醉前后血压(BP)、心率(HR)、麻醉起效时间、麻醉维持时间、术后疼痛程度及不良反应发生率等指标。结果麻醉后10min,两组患者的SBP、DBP及HR水平显著低于麻醉前,观察组患者的SBP、DBP及HR水平明显高于对照组(P<0.05)。观察组患者运动和感觉神经阻滞起效时间短,运动和感觉神经阻滞维持时间长,两组数据相比较,差异有统计学意义(P<0.05)。观察组不良反应发生率(15.00%)明显低于对照组(46.67%)(P<0.05)。结论超声引导下外周神经阻滞麻醉应用于孕妇下肢骨折麻醉效果满意,对患者所产生的副作用比较小,安全可靠,可提高患者的手术质量及就医感受。Objective To observe the clinical efficacy and safety of ultrasound-guided peripheral nerve block anesthesia in the operation of lower limb fracture in pregnant women.Methods In this study,20 pregnant women with lower limb fractures who received surgical treatment in the orthopedics Department of Baiyin Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to October 2022 were selected as the observation group,mainly patellar fracture and external malleolus fracture.Ultrasound-guided nerve block anesthesia was used for surgery.At the same time,30 non-pregnant patients with patellar and lateral malleolus fractures were randomly selected as the control group.Blood pressure(BP),heart rate(HR),onset time of anesthesia,maintenance time of anesthesia,postoperative pain degree and incidence of adverse reactions were observed in two groups.Results After 10 minutes of anesthesia,the levels of SBP,DBP and HR in both groups of patients were significantly lower than before anesthesia,while the levels of SBP,DBP and HR in the observation group were significantly higher than those in the control group(P<0.05).In the observation group,the onset time of movement and sensation was short,and the duration of movement and sensory block was long,and the difference between the two groups was statistically significant(P<0.05).The incidence of adverse reactions in observation group(15.00%)was significantly lower than that in control group(46.67%)(P<0.05).Conclusion Ultrasound-guided peripheral nerve block anesthesia in pregnant women with lower extremity fracture has a satisfactory effect,the side effects on patients are relatively small,safe and reliable,improve the quality of surgery and medical experience of patients.
