车站地区献血相关血管迷走神经反应影响因素分析

摘　　要：目的讨论和分析影响天津车站地区全血献血者献血相关血管迷走神经反应(donation'relatedvasovagal reaction,DRVR)的相关因素,为降低车站地区DRVR的发生率提供参考依据。方法随机抽取2020年1月-2021年12月天津车站地区全血献血者18326例(人次)作为研究对象,对发生DRVR的献血者资料进行分析,统计DRVR的发生与性别、年龄、献血史、体重、献血量、精神紧张、睡眠情况、空腹/饥饿、献血体位及职业等因素的关系,进行单因素及多因素Logistic分析。结果通过现场记录及电话回访,结合DRVR诊断标准,18326例献血者DRVR发生168例,发生率为0.92%。单因素及多因素Logistic分析结果表明:天津车站地区全血献血者DRVR发生率与性别、体重、献血量无统计学意义(P>0.05);与年龄、献血史、精神紧张、睡眠不足、空腹/饥饿、献血体位、职业具有统计学意义(P<0.05)。结论车站地区全血献血者献血相关血管迷走神经反应发生率较高,应加强献血前征询,献血过程中要特别注意空腹/饥饿、睡眠不足、精神紧张以及初次、低龄的献血者,采取针对性措施预防DRVR发生,保障献血者安全。Objective To explore the influence of multifactor logistic analysis on blood donation related vagal reaction(DRVR)of blood donors in Tianjin station area,so as to provide a reference for reducing the incidence of DRVR in the station area.Methods 18326 blood donors in Tianjin east railway station from January 2020 to December 2021 were selected as the research objects.The data of blood donors with DRVR were analyzed,and the relationship between DRVR and gender,age,blood donation times,weight,occupation,blood donation volume,mental stress,lack of sleep,fatigue,fasting/hunger,blood donation position and other factors was counted,Single factor and multi factor Logistic analysis were carried out.Results Through on-site recording and telephone return visit records,according to the DRVR diagnostic criteria,168 cases of DRVR occurred in 18326 blood donors,with a incidence rate of 0.92%.The results of univariate and multivariate Logistic analysis showed that the incidence of DRVR in whole blood donors in Tianjin Railway Station area was not statistically significant with sex,weight and blood donation(P>0.05),and it was statistically significant with age,blood donation history,mental stress,lack of sleep,fasting/hunger,blood donation position,and occupation(P<0.05).Conclusions The incidence of vasovagus nerve reactions related to blood donation in whole blood donors in the station area is high,and pre-donation consultation should be strengthened,and special attention should be paid to fatigue,fasting/hunger,lack of sleep,mental stress,and first-time and young blood donors during the blood donation process,and targeted measures should be taken to prevent the occurrence of DRVR and ensure the safety of blood donors.
