丁济万湿温治疗八法探析

摘　　要：《丁济万医案》记录了孟河医派代表人物丁济万治疗湿温病的经验。文章从疏风宣化、和畅中都、清透宣化、清宣和解、清热导滞、清透存阴、开窍息风、益气养阴八个方面对其湿温治法特色进行阐述。疏风宣化法用于湿温兼风之治,清透宣化、清宣和解、清热导滞三法分用于湿温病在三阳的证治,从阳入阴用清透存阴、开窍息风二法,善后恢复当益气养阴,和畅中都法贯穿湿温治疗始终。丁济万的辨治经验对现今湿温时病及临床各科湿热类疾病的诊疗具有重要的指导意义与实用价值。The Ding Jiwan Medical Records notes down the experience of Ding Jiwan,a representative of the Menghe medical school,in treating damp-warm diseases.In this article,his treatment characteristics of damp-warm diseases are elaborated on from the following 8 aspects:dispelling wind and promoting qi-flow to disperse dampness,harmonizing and promoting the spleen and stomach,clearing heat and promoting qi circulation to disperse dampness,clearing heat and promoting qi movement to harmonize Shaoyang,clearing heat to remove food stagnation,clearing heat and promoting qi circulation to nourish yin,inducing resuscitation to calm endogenous wind,and benefiting qi to nourish yin.Among them,the method of dispelling wind and promoting qi-flow to disperse dampness is used for the treatment of syndrome of damp-warm combined with endogenous wind;the three methods of clearing heat and promoting qi circulation to disperse dampness,clearing heat and promoting qi movement to harmonize Shaoyang,clearing heat to remove food stagnation are respectively used for the treatment of damp-warm diseases involving the three yang channels;the two methods of clearing heat and promoting qi circulation to nourish yin,inducing resuscitation to calm endogenous wind are used for the treatment of damp-warm diseases caused by heat in Yangming transforming into dampness.For their later recovery,the method of benefiting qi to nourish yin should be employed,and the method of harmonizing and promoting the spleen and stomach should run through their whole treatment course.Ding Jiwan's experience in differentiating and treating the damp-warm diseases is of great guiding significance and practical value for the diagnosis and treatment of damp-warm epidemics and related damp-warm diseases in various clinical departments.
