摘　　要：慢性咳嗽是呼吸科常见的疾病,严重影响患者的身心健康。王檀认为本病的病因是“脾肾虚冷,邪留于肺”,病机关键为“肺气不清”。临床常见咳嗽迁延不愈2个月以上,干咳或咳白痰,平素可见畏寒肢冷,乏力,纳呆,便溏,小便清长等症,劳累或感寒后加重。治疗上以“温补脾肾,肃清肺气”为基本原则,以附子理中丸为基础方剂方进行加减化裁,方药组成具有温清同用、标本兼治的特点,达到改善全身虚冷状态,肃清肺中邪气作用。王檀临证整体审查,以温补脾肾,肃清肺气为本,兼顾化痰散瘀、清心胃火治疗慢性咳嗽,标本兼治,疗效颇佳。Chronic cough is a common respiratory disease,which seriously affects the physical and mental health of patients.Professor Wang Tan,a nationally renowned traditional Chinese medicine practitioner,believes that the etiology of this disease is"the deficient cold in the spleen and kidney,the pathogenic factors in the lungs",and its key pathogenesis is"unclear lung qi".Its common clinical symptoms include prolonged cough for more than 2 months,dry cough or coughing with white phlegm,and common symptoms such as fear of cold and cold limbs,fatigue,anorexia,loose stools,and clear and long urine,which may worsen after a fatigue or a cold.In its treatment,the basic principle is"warming and tonifying the spleen and kidney,purifying and clearing lung qi",and Fuzi Lizhong Wan is used as a basic prescription,in which the herbs have the medication characteristics of both warming the spleen and kidney and clearing lung qi,and treating both the root cause and manifestations of the disease,improving the body's deficiency and cold state and purifying and clearing away the evil qi in the lungs.In clinic,Professor Wang Tan adopts a holistic syndrome differentiation,giving priority to warming and tonifying the spleen and kidney,purifying and clearing the lung qi,as well as resolving phlegm and blood stasis,clearing fire in the heart and stomach,achieving a good therapeutic effect.
