针刺联合十味温胆汤治疗卒中后心胆气虚型失眠

摘　　要：目的探究针刺结合十味温胆汤加减治疗卒中后心胆气虚型失眠的临床效果。方法选取成都中医药大学附属资阳医院2019年5月-2022年5月接诊的94例卒中后心胆气虚型失眠患者为研究对象。随机数字表法分为对照组(针刺治疗)和治疗组(针刺联合十味温胆汤加减治疗),各47例。统计2组患者治疗前后睡眠潜伏期、惊醒次数、睡眠总时长、睡眠效率、统计治疗后的临床疗效。采用匹兹堡睡眠质量指数(PSQI)对睡眠质量进行评价。治疗前后采用酶联免疫吸附法检测肿瘤坏死因子(TNF-α),白介素6(IL-6),脑源性神经营养因子(BDNF)水平。统计治疗期间恶心、呕吐、便秘、皮疹等不良反应的发生情况。结果治疗组的睡眠潜伏期短于对照组,惊醒次数少于对照组,睡眠总时长、睡眠效率、治疗总有效率高于对照组(P<0.05)。治疗组PSQI各项得分和PSQI总分均低于对照组(P<0.05)。治疗后治疗组的TNF-α、IL-6水平低于对照组,脑源性神经营养因子水平高于对照组(P<0.05)。2组的不良反应总发生率差异无统计学意义(P>0.05)。结论针刺联合十味温胆汤加减可以提高卒中后失眠患者的睡眠质量,临床疗效较佳,且安全性较高。Objective To explore the clinical effect of acupuncture combined with modified Shiwei Wendan Decoction in the treatment of insomnia with the syndrome of qi deficiency of heart and gall after stroke.Methods A total of 94 patients with the syndrome of qi deficiency of heart and gall after stroke admitted to our hospital from May 2019 to May 2022 were selected as the research subjects,and were randomly divided into a control group(treated with acupuncture)and a Shiwei Wendan Decoction group(treated with acupuncture combined with modified Shiwei Wendan Decoction),with 47 cases in each group.The sleep latency,wake-up times,total sleep time and sleep efficiency were counted in the two groups before and after treatment.The clinical efficacy after treatment was counted.The Pittsburgh Sleep Quality Index(PSQI)was used to evaluate their sleep quality.Before and after treatment,enzyme linked immunosorbent assay was used to detect the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and brain derived neurotrophic factor(BDNF).The incidence of adverse reactions such as nausea,vomiting,constipation and rash was counted during the treatment period.Results After treatment,the sleep latency of the Shiwei Wendan Decoction group was shorter than that of the control group,the number of wake-ups was less than that of the control group,and the total sleep duration,sleep efficiency and total effective rate were higher than those of the control group(P<0.05).After treatment,the scores of PSQI and the total score of PSQI in the Shiwei Wendan Decoction group were lower than those in the control group(P<0.05).After treatment,in the Shiwei Wendan Decoction group,the levels of TNF-αand IL-6 were lower than those of the control group,while the level of BDNF was higher than that of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the control group and the Shiwei Wendan Decoction group(P<0.05).Conclusion Acupuncture combined with modified Shiwei Wendan Decoction can
