摘 要:目的:建立以无人自助发药系统为核心的智慧化发热药房,为新冠疫情常态化管理下发热药房的持续改进提供新策略。方法:收集北京大学第三医院医院信息系统发热药房改进前后年份第一季度同期处方信息。采用计划-执行-检查-处理(plan-do-check-action, PDCA)循环法,评价药房持续改进对疫情防控管理的影响;采用定量法,对同期人力成本、人工效率、调配差错等指标进行分析。结果:对发热药房服务模式的改进,明显改善疫情管理下发热药房的感染防控体系。相较于同期第一季度,月度平均班次显著下降(P<0.01),调配差错数同比明显降低。在患者人数显著增加的前提下,患者平均取、候药时间相当,且10 min内完成退药的患者比例明显提升,差异有统计学意义(P=0.002)。结论:智慧化发热药房的建立,能有效减少疫情的传播风险,提高人工效率,减少调配差错,实现药学服务精细化管理,为医院药学高质量发展提供重要借鉴和参考。OBJECTIVE To establish an intelligent fever pharmacy with a self-service dispensing system,and to provide a new strategy for the continuous improvement of fever pharmacies under the epidemic management.METHODS Using the hospital information system(HIS)of Peking University Third Hospital,we collected prescribing data before and after the adjustment of fever pharmacy in the first quarter of adjacent years.The plan-do-check-action(PDCA)cycle method was performed to evaluate the impact on the epidemic management.The quantitative indices were used to analyze human cost,labor efficiency and dispensing errors.RESULTS The epidemic management system was significantly elevated through the improvement of the fever pharmacy model.The monthly shifts were significantly dropped compared with those during the same period in the first quarter(P<0.01).Moreover,dispensing errors were decreased year-on-year.Although there was a substantial increase in the number of patients,the average waiting time was identical,and the proportion of patients who completed the process within 10 minutes was significantly improved(P=0.002).CONCLUSION The establishment of intelligent fever pharmacy effectively reduces the risk of epidemic transmission,improves labor efficiency,reduces dispensing errors,realizes the refined management of pharmacy services,and provides essential reference for the high-quality development of hospital pharmacy.
