雷火灸治疗蠕形螨相关睑板腺功能障碍型干眼的疗效观察

摘　　要：目的观察雷火灸治疗蠕形螨相关睑板腺功能障碍(MGD)型干眼的临床疗效。方法纳入2021年10月—2022年6月济南明水眼科医院诊治的蠕形螨相关MGD型干眼患者90例(180只眼),随机分为治疗组和对照组,每组45例(90只眼)。对照组采用4-松油醇眼部除螨棉片+睑板腺按摩+玻璃酸钠滴眼液治疗。治疗组将对照组4-松油醇眼部除螨棉片改为雷火灸治疗。2组均治疗1个月。分别于治疗前、治疗后检测受试者的眼表疾病指数(OSDI)评分、泪膜破裂时间(BUT)及蠕形螨的数量。记录以上数据并做统计分析。结果2组治疗前OSDI评分、BUT、MGS及蠕形螨的数量比较,差异均无统计学意义(P>0.05)。(1)OSDI评分:治疗前后比较,2组OSDI评分均较治疗前降低(t治疗组=25.751,t对照组=10.964,均P=0.000)。治疗后2组间比较,治疗组OSDI评分较对照组降低(t=3.982,P=0.000)。(2)BUT:治疗前后比较,2组BUT均较治疗前增加(t治疗组=19.346,t对照组=17.407,均P=0.000)。治疗后2组间比较,治疗组BUT较对照组提高(t=4.477,P=0.000)。(3)蠕形螨计数:治疗前后比较,2组蠕形螨计数均较治疗前减少(t治疗组=9.093,t对照组=6.899,均P=0.000)。治疗后2组间比较,治疗组蠕形螨计数较对照组减少(t=4.108,P=0.000)。以上比较,差异均有统计学意义。结论雷火灸治疗可以有效改善蠕形螨相关MGD型干眼患者的症状,值得临床推广。OBJECTIVE To observe the clinical effect of thunder fire moxibustion on dry eye with meibomian gland dysfunction(MGD)related to demodex infection.METHODS A total of 90 patients(180 eyes)of dry eye with MGD related to demodex infection who were treated in Jinan Mingshui Eye Hospital from October 2021 to June 2022 were included and randomly divided into treatment group(TG)and control group(CG),with 45 cases(90 eyes)in each group.The patients in the control group were treated with four-terpinol eye anti-mite cotton piece,combined with meibomian gland massage and sodium hyaluronate eye drops.While in the treatment group,the four-terpinol eye anti-mite cotton piece was replaced with thunder fire moxibustion.All patients of both groups were treated for one month.The ocular surface disease index(OSDI)score,tear film rupture time(BUT)and the number of demodex were measured before and after treatment.The above data was recorded and statistical analysis was conducted.RESULTS There was no statistically significant difference in OSDI scores,BUT,MGS and the number of demodex between the two groups before treatment(P>0.05).(1)OSDI scores:Compared before and after treatment,the OSDI scores in both groups decreased compared to those before treatment(tTG=25.751,tCG=10.964,both P=0.000).After treatment,when compared between the two groups,the OSDI scores in the treatment group were lower than those of the control group(t=3.982,P=0.000).(2)BUT:Compared before and after treatment,both groups showed an increase in BUT compared to those before treatment(tTG=19.346,tCG=17.407,both P=0.000).After treatment,when compared between the two groups,the treatment group showed an increase in BUT compared to the control group(t=4.477,P=0.000).(3)The number of demodex:Compared before and after treatment,the demodex count in both groups decreased compared to those before treatment(tTG=9.093,tCG=6.899,both P=0.000).After treatment,when compared between the two groups,the demodex count in the treatment group decreased compared to the control group(t
