摘　　要：目的探讨光照疗法联合保守氧疗对降低ICU机械通气患者谵妄发生率及改善睡眠质量的效果。方法选择2020年3月—2022年3月医院ICU机械通气的120例患者为研究对象,按照组间基本资料均衡可比的原则分为3组,各40例。A组给予100 Lux光照强度的安慰光照治疗,B组给予10000 Lux光照强度的LED治疗灯治疗,C组给予10000 Lux光照强度的LED治疗灯联合保守氧疗治疗。对比3组患者谵妄发生情况、ICU结局、睡眠质量及舒适度情况。结果C组谵妄发生率低于A组、B组,谵妄持续时间短于A组、B组,差异具有统计学意义(P<0.05);3组ICU住院时间、ICU病死率与非计划性拔管发生率比较,差异均无统计学意义(P>0.05);出ICU时C组睡眠质量各维度评分、总均分均低于A组、B组,差异具有统计学意义(P<0.05);出ICU时C组舒适度各维度评分、总分均高于A组和B组,差异具有统计学意义(P<0.05)。结论10000 Lux光照疗法联合保守氧疗可降低ICU机械通气患者谵妄发生率,缩短谵妄持续时间,改善睡眠质量,提升患者舒适度。Objective To explore the effect of light therapy combined with conservative oxygen therapy on reducing the incidence of delirium and improving sleep quality in ICU mechanically ventilated patients.Methods A total of 120 patients with mechanical ventilation in the ICU of the hospital from March 2020 to March 2022 were selected as the research subjects.They were divided into three groups based on the principle of balanced and comparable basic data between the groups,with 40 patients in each group.Group A received comfort light therapy with a light intensity of 100 Lux,group B received LED therapy with a light intensity of 10,000 Lux,and group C received LED therapy with a light intensity of 10,000 Lux combined with conservative oxygen therapy.The incidence of delirium,ICU outcomes,sleep quality,and comfort were compared among three groups of patients.Results The incidence of delirium in group C was lower than that in group A and group B,and the duration of delirium was shorter than that in group A and group B,the differences were statistically significant(P<0.05).There was no statistically significant difference in the length of ICU stay,ICU mortality rate,and unplanned extubation incidence among the three groups(P>0.05).When leaving the ICU,the sleep quality scores and total average scores of group C were lower than those of group A and group B,the diff erences were statistically signifi cant(P<0.05).When leaving the ICU,the comfort scores and total average scores of group C were higher than those of group A and group B,the differences were statistically significant(P<0.05).Conclusion The combination of 10,000 Lux light therapy and conservative oxygen therapy can reduce the incidence of delirium in ICU mechanically ventilated patients,shorten the duration of delirium,improve sleep quality,and enhance patient comfort.
