摘　　要：目的 探讨海岛妇女早期复发性流产(ERSA)血红素加氧酶-1(HO-1)、单核细胞趋化因子-1(MCP–1)、核转录因子(NF-κB)的表达及意义。方法 选取30例ERSA患者与30例正常妊娠期因非计划妊娠要求终止而就诊的患者作为研究对象,分别作为ERSA组和对照组。分别采用q PCR、Western blot法检测,比较两组患者绒毛蜕膜组织中HO-1、MCP-1、NF-κB的表达及含量变化,采用ROC曲线分析各指标预测ERSA发病的价值。结果 ERSA组绒毛蜕膜组织中HO-1、MCP-1、NF-κB mRNA水平均高于对照组,差异均有统计学意义(t分别=43.22、43.43、8.28,P均<0.05);ERSA组绒毛蜕膜组织中HO-1、MCP-1、NF-κB蛋白的表达均高于对照组,差异均有统计学意义(t分别=28.35、13.37、9.54,P均<0.05)。绒毛蜕膜组织中HO-1、MCP-1、NF-κB mRNA水平联合检测ERSA的灵敏度和特异度最高,分别为72.41%和90.00%,AUC为0.84;绒毛蜕膜组织中HO-1、MCP-1、NF-κB蛋白水平联合检测ERSA的灵敏度和特异度最高,分别为86.67%和86.66%,AUC为0.85。结论 ERSA患者绒毛蜕膜组织中的HO-1、MCP-1、NF-κB表达与正常妊娠妇女存在明显差异,其可作为预测ERSA发生的重要因子,且联合三者检测诊断效能更高。Objective To investigate the expression and significance of heme oxygenase-1(HO-1),monocyte che⁃moattractant-1(MCP-1)and nuclear factor kappa B(NF-κB)in early recurrent abortion sponteaneous(ERSA)of is⁃land women.Methods Thirty patients with ERSA and 30 patients with unplanned pregnancy termination in normal pregnancy were selected as the research objects,respectively as ERSA group and control group.QPCR and Western blot were used to detect and compare the expression and content changes of HO-1,MCP-1 and NF-κB in decidua tissue of two groups,and ROC curve was used to analyze the value of each index in predicting the onset of ERSA.Results The mRNA levels of HO-1,MCP-1 and NF-κB in chorionic decidua tissue in ERSA group were higher than those in control group(t=43.22,43.43,8.28,P<0.05).The protein expressions of HO-1,MCP-1 and NF-κB in chorionic decidua tissue in ERSA group were higher than those in control group(t=28.35,13.37,9.54,P<0.05).The sensitivity and specificity of HO-1,MCP-1 and NF-κB mRNA in chorionic decidua tissue were 72.41%and 90.00%,respectively,and the AUC was 0.84.The sensitivity and specificity of HO-1,MCP-1 and NF-κB protein in chorionic decidua tissue were 86.67%and 86.66%,respectively,and the AUC was 0.85.Conclusion The expression of HO-1,MCP-1 and NF-κB in chorionic decidua tissue of patients with ERSA is significantly different from that of normal pregnant women,which can be used as an important factor to predict the occurrence of ERSA,and the combined detection and diagnosis of these three factors is more effective.
