P波切迹临床意义探讨

摘　　要：目的　本文对P波切迹临床意义进行探讨。方法　本文共记录 336例心电图 ,其中 16 3例为健康对照组 ,173例为高血压组 ,高血压组中随机选择 4 8例 ,按P波时限分两组 :高血压A组 (P波时限 <0 11秒 )及高血压B组 (P波时限≥ 0 11秒 ) ,分析高血压A组、B组心脏临床异常事件检出率及对高血压A组、B组及正常人组P波时限、峰间距、麦氏指数、PtfV11均值检验。结果　健康对照组P波切迹阳性率为 2 0 % ,而高血压组为 5 3% ,高血压A组心脏临床异常事件检出率为 19% ,高血压B组为70 % (P <0 0 1) ,均值检验说明PTfV1对宽时限P波切迹有一定参考价值。结论　宽时限P波切迹提示心脏临床异常事件发生可能 ,按P波时限可分Ⅰ°、Ⅱ°及Ⅲ°P波切迹 ,除左房大外 ,尚有不完全性左房内传导阻滞可能。Objective To analyses notched P wave in clinic.Method 336 cases' ECG were recorded,173 cases were in the hypertension group,163 cases in the health contrast group.48 patients were chosen from the hypertension group at random,and divided into two parts according to the duration of P wave.That is,in group A, P <0 11s,while in group B, P ≥0 11s.The positive rate of notched P wave,the detected rate of clinical abnormal event of heart,and the means of several parameters,such as the duration of P wave,the span of the two peak of the P wave,P/P-R,P terminal force of V 1(PtfV 1),were tested.Result In the hypertension group the positive rate of notched P wave was 53%,in the health contrast group the positive rate of notched P wave was 20%.The detected rate of clinical abnormal event of heart in group A was 19%,and 70% in group B( P <0 01).The means of several parameters of the three groups (group A,group B and the health contrast group),indicated that long duration notched P wave might imply the possibility of clinical abnormal event of heart,and PtfV 1 is related to long duration notched P wave.Conclusion Notched P wave might be differentiated into Ⅰ°,Ⅱ°,Ⅲ° according to the duration of P wave and incomplete left interatrial block might be the cause of notched P wave,besides left atrial enlargement.
