摘 要:目的探讨升降润肠方治疗儿童功能性便秘(functional constipation,FC)的可能作用机制。方法将48只SD大鼠随机分为空白组、模型组、乳果糖组、升降润肠方组,每组12只,除空白组外,其余36只采用盐酸小檗碱灌胃建立SD大鼠FC模型;造模成功后,空白组、模型组均予生理盐水1 mL/次,2次/天,灌胃;乳果糖组予乳果糖口服溶液1 mL/次,第2次予生理盐水1 mL/次,灌胃;升降润肠方组予中药液1 mL/次,2次/天,灌胃;四组均灌胃14天。疗程结束后,各组大鼠雌、雄各随机抓取3只,脱颈处死,通过计算胃排空率、碳墨推进率、记录结肠内存留粪粒数来评价大鼠胃肠动力;其余大鼠麻醉后剖腹取近端结肠组织,采用免疫组化检测水通道蛋白3(aquaporin-3,AQP3)、水通道蛋白9(aquaporin-9,AQP9)在结肠组织分布及表达情况,实时荧光定量聚合酶链反应(quantitative real-time polymerase chain reaction,qRT-PCR)进一步检测其mRNA水平,蛋白印迹(western blot,WB)检测其蛋白表达情况。结果与空白组相比,模型组的胃排空率、碳墨推进率降低,存留粪粒数增多,结肠黏膜AQP3 mRNA及蛋白表达上升,AQP9 mRNA及蛋白表达下降(P<0.05)。与模型组相比,乳果糖组的胃排空率差异无统计学意义(P>0.05),但碳墨推进率增加,存留粪粒数减少,结肠黏膜AQP3 mRNA及蛋白表达下降(P<0.05),AQP9 mRNA及蛋白表达差异无统计学意义(P>0.05);与模型组相比,升降润肠方组的胃排空率、碳墨推进率增加,存留粪粒数减少,结肠黏膜AQP3 mRNA及蛋白表达下降,AQP9 mRNA及蛋白表达上升(P<0.05)。与乳果糖组相比,升降润肠方组的胃排空率增加(P<0.05),碳墨推进率、存留粪粒数、结肠黏膜AQP3 mRNA及蛋白表达差异无统计学意义(P>0.05),AQP9 mRNA及蛋白表达上升(P<0.05)。结论升降润肠方治疗儿童FC的作用机制与其能促进胃肠动力,下调近端结肠黏膜AQP3及上调近端结肠黏膜AQP9的表达相关。Objective To explore the possible mechanism of Shengjiang Runchang formula in the treatment of children functional constipation.Methods 48 SD rats were randomly divided into the blank group,the model group,the lactulose group and the Shengjiang Runchang formula group,with 12 rats in each group.Except the blank group,the remaining 36 rats were given berberine hydrochloride intragastric administration to establish functional constipation rats model.After the model was sucessfully established,all the rats in both the blank group and the model group were given normal saline 1mL/time,twice a day,by intragastric administration.The lactulose group was given lactulose oral solution 1mL/time and normal saline 1mL/time for the second time,by intragastric administration.The Shengjiang Runchang Formula group was given Chinese medicine liquid 1mL/time,twice a day,by intragastric administration.The four groups were given gavage for 14 days.At the end of the treatment course,3 male and female rats in each group were randomly captured and then killed by neck removal.Gastrointestinal motility of rats was evaluated by calculating gastric emptying rate,the carbon ink propulsion rate,and the number of remaining fecal particles in the colon.The proximal colon tissues of the remaining rats were taken after anesthesia.The distribution and expression of aquaporin-3(AQP3)and aquaporin-9(AQP9)in the colon tissues were detected by immunohistochemistry.The mRNA level was further detected by RT-PCR,and the protein expression was detected by WB.Results Compared with the blank group,the gastric emptying rate and the carbon ink propulsion rate were decreased in the model group,but the number of remaining fecal particles were increased;AQP3 mRNA and protein expression were increased,AQP9 mRNA and protein expression were decreased(P<0.05).Compared with the model group,there was no significant difference in the gastric emptying rate in the lactulose group(P>0.05),but the carbon ink propulsion rate was increased and the number of remaining fecal pa
