摘 要:目的 探讨标准轴位弥散加权成像(DWI)、薄层冠状位DWI及多平面重建(multiplanar reconstruction mode, MPR)技术对急性脑干梗死的诊断能力分析。方法 选取45例临床诊断为急性脑干梗死且行常规轴位DWI、薄层冠状位DWI检查的患者,并进行薄层冠状位DWI的MPR后处理,记录患者入院时常规轴位DWI、轴位联合薄层冠状位DWI+MPR图像诊断为急性脑干梗死的阳性及阴性病例数。结果 45例中轴位DWI表现为阳性者32例(71.1%),阴性者13例(28.9%);常规轴位DWI+薄层冠状位DWI+MPR表现为阳性者41例(91.1%),阴性者4(8.9%)例;常规轴位DWI+薄层冠状位DWI+MPR对急性脑干梗死的诊断能力高于常规轴位DWI,差异有统计学意义(χ~2=5.87,P=0.029)。结论 常规轴位DWI联合薄层冠状位DWI及MPR有助于提高临床医师对急性脑干梗死的诊断能力,具有较好的临床应用价值。Objective To investigate the efficiency of axial,thin-layer coronal diffusion weighted imaging and multiplanar reconstruction mode in diagnosing acute brainstem infarctions.Methods 45 patients with clinical diagnosis consistent with acute brain stem infarction,who received magnetic resonance imaging(DWI),including axial and coronal DWI with multiplanar reconstruction mode,were retrospectively analysed.The numbers of positive and negative cases of acute brain stem infarction were recorded respectively in axial DWI alone or in combinations with thin-layer coronal DWI by multiplanar reconstruction mode.Results Of those 45 patients,32(71.1%)were positive and 13(28.9%)were negative by axial DWI;41 patients(91.1%)were positive and 4(8.9%)were negative by axial DWI+thin-layer coronal DWI+MPR.The positive rate of acute brain stem infarction in axial DWI+thin-layer coronal DWI+MPR was significantly higher than that on axial DWI alone(2=5.87,P=0.029).Conclusion Axial DWI combined with thin-layer coronal diffusion-weighted imaging with multiplanar reconstruction mode improves the detection rate of acute brain stem infarction and,therefore,appears to be clinically useful.
