摘  要:目的探讨振幅整合脑电图(aEEG)联合神经元烯醇化酶(NSE)、S100钙结合蛋白B(S100B)、脑干听觉诱发电位(BAEP)及新生儿行为神经评定(NBNA)在胆红素脑病中的变化及诊断价值。方法将2020年8月至2021年8月成都市金牛区妇幼保健院收治的胆红素脑病10例纳入脑损伤组,同期将该院收治的新生儿高胆红素血症无脑损伤新生儿76例纳入非脑损伤组。比较两组中aEEG、NSE、S100B、BAEP及NBNA的变化,并绘制ROC曲线,评估其诊断价值。结果脑损伤组中重度aEEG的比例显著高于非脑损伤组,差异有统计学意义(P<0.05)。此外,脑损伤组的NSE、S100B、BAEP及NBNA的比例也均显著高于非脑损伤组,差异均有统计学意义(P<0.05)。预测脑损伤的诊断性能分析显示,aEEG、NSE、S100B、BAEP及NBNA的AUC分别为0.896、0.734、0.720、0.750、0.550。联合5个因素的AUC为0.993,提示诊断价值很好。结论aEEG联合NSE、S100B、BAEP及NBNA对胆红素脑病的诊断价值均较好。Objective To explore the changes and diagnostic value of amplitude integrated electroencephalogram(aEEG)combined with neuron-specific enolase(NSE),S100 calcium binding protein B(S100B),brainstem auditory evoked potential(BAEP)and neonatal behavioral neurological assessment(NBNA)in bilirubin encephalopathy.Methods From August 2020 to August 2021,a total of 10 cases of bilirubin encephalopathy admitted to Jinniu Maternal and Child Health Hospital of Chengdu were included in the brain injury group,and 76 cases of neonatal hyperbilirubinemia without brain injury admitted to this hospital during the same period were included in the non-brain injury group.The changes of aEEG,NSE,S100B,BAEP and NBNA were compared between the two groups and ROC curves were drawn to evaluate their diagnostic value.Results The proportion of severe aEEG in the brain injury group was significantly higher than that in the non-brain injury group,the difference was statistically significant(P<0.05).In addition,the proportion of NSE,S100B,BAEP and NBNA in the brain injury group were also significantly higher than those in the non-brain injury group,the difference was statistically significant(P<0.05).The diagnostic performance analysis for predicting brain injury showed that the AUC of aEEG,NSE,S100B,BAEP,and NBNA were 0.896,0.734,0.720,0.750,and 0.550,respectively.The AUC of the five combined factors was 0.993,suggesting a good diagnostic value.Conclusion aEEG combined with NSE,S100B,BAEP and NBNA has a good diagnostic value for bilirubin encephalopathy.
