摘　　要：目的探讨3.0T MR实时引导下微波消融(MWA)治疗肝脏寡转移瘤的可行性及短期疗效。方法回顾性分析接受3.0T MR引导下MWA的30例肝脏寡转移瘤患者共44个病灶,观察并记录技术成功率、平均手术时间、平均消融时间及并发症。以改良实体瘤疗效评价标准(mRECIST)(2020版)评价短期疗效,计算局部控制率。结果44个病灶均操作成功,技术成功率100%。平均手术时间(94.37±17.24)min,单个病灶平均消融时间(11.34±4.37)min,平均消融功率(42.84±3.30)W。1例患者术中出现胆管出血,30例患者在随访期间未发生大出血、肝衰竭、感染、死亡等严重并发症或不良事件。平均随访时间(15.93±9.38)个月,1个病灶随访期间出现局部复发,局部控制率97.7%(43/44)。结论3.0T MR实时引导下MWA治疗肝脏寡转移瘤安全、可行,且具有良好的肿瘤控制率。Objective To investigate the feasibility and short-term efficacy of 3.0T MR real-time guided microwave ablation(MWA)in the treatment of hepatic oligometastases.Methods Thirty patients with hepatic oligometastases(44 lesions total)who underwent 3.0T MR guided MWA were analyzed retrospectively.The success rate,average operation time,average ablation time and complications were observed and recorded.The short-term efficacy was evaluated according to the modified response evaluation criteria in solid tumor(mRECIST)(2020 version),and the local control rate was also calculated.Results All 44 lesions were operated successfully,with a success rate of 100%.The average operation time was(94.37±17.24)min,with the average ablation time per lesion of(11.34±4.37)min,and the average ablation power was(42.84±3.30)W.One patient had intraoperative bile duct bleeding,and no serious complications or adverse events such as major bleeding,liver failure,infection,or death occurred in 30 patients during the follow-up period.The average follow-up time was(15.93±9.38)months,and the local control rate was 97.7%(43/44).Only 1 lesion had local recurrence during the follow-up period.Conclusion 3.0T MR real-time guided MWA in the treatment of hepatic oligometastases is safe and feasible,and has a good tumor control rate.
