摘　　要：谢晶日教授将慢性萎缩性胃炎病机演变概括为“一基三段五期”。“一基”(一个基础),即肝脾不和、脾胃虚弱;“三段”(三大阶段),即痰(湿)、瘀、毒;“五期”(五个分期),即痰湿内阻期、湿瘀互结期、痰瘀互结期、痰瘀挟毒期、瘀毒炽盛期。遵循“微观辨证、五脏同治、循环递进、镜药结合”四总则,以“一基三段五期”理论指导临床,使治疗有的放矢。欣胃方用于治疗慢性萎缩性胃炎属痰瘀互结者,可有效减轻胃黏膜炎症,延缓甚至逆转肠上皮化生进展,有效防治胃癌。Professor Xie Jingri summarizes the pathogenesis evolution of chronic atrophic gastritis as“one base,three phases and five stages”.“One base”means incoordination between liver and stomach,deficiency of spleen and kidney;“three phases”means phlegm(dampness),stasis,and toxin;“five stages”means phlegm-damp internal obstruction,intermingled dampness and blood stasis,intermingled phlegm and blood stasis,phlegm-stasis with toxin,blazing stasis-toxin.The treatment should follow the four general rules of“microcosmic syndrome differentiation,simultaneous treatment of five organs,circular progression,and combination of medication and gastroscopy”,and the theory of“one base,three phases and five stages”should be used to guide clinical treatment,so as to be more targeted.Xinwei Prescription is used to treat chronic atrophic gastritis with intermingled phlegm and blood stasis,which can effectively reduce gastric mucosal inflammation,delay or even reverse intestinal metaplasia,and effectively prevent the occurrence of gastric cancer.
