循经针刺推拿法治疗小儿痉挛型脑瘫的临床效果

摘　　要：目的探讨循经针刺推拿法治疗小儿痉挛型脑瘫的临床效果。方法选取2020年1月至2022年4月河南中医药大学第一附属医院收治的90例痉挛型脑瘫患儿,按照随机数字表法将其分为对照组与研究组,各45例。两组均予以常规康复治疗,对照组在此基础上予以A型肉毒素治疗,研究组在对照组基础上予以循经针刺推拿法治疗。两组治疗3个月后,评估临床疗效,比较治疗前后临床痉挛指数(CSI)、精细运动发育量表(PDMS-FM)、粗大运动功能(GMFM)及改良Ashworth分级法(MAS)评分。结果研究组临床疗效优于对照组(P<0.05)。治疗后,两组PDMS-FM及GMFM评分较治疗前升高,且研究组高于对照组(P<0.05)。治疗后,两组上下肢MAS评分较治疗前降低,且研究组低于对照组(P<0.05)。治疗后两组阵挛、肌张力、腱反射评分及CSI总评分较治疗前降低,且研究组低于对照组(P<0.05)。结论循经针刺推拿法治疗小儿痉挛型脑瘫效果较佳,能有效减轻患儿肌肉痉挛状态,改善患儿运动功能。Objective To explore the clinical effect of acupuncture and massage along the meridian in the treatment of spastic cerebral palsy in children.Methods A total of 90 children with spastic cerebral palsy admitted to the First Affiliated Hospital of Henan University of Chinese Medicine from January 2020 to April 2022 were selected and they were divided into the control group and the study group according to the random number table,with 45 cases in each group.Both groups were treated with routine rehabilitation.The control group was treated with Botulinum Toxin A on this basis,and the study group was treated with acupuncture and massage along meridians on the basis of the control group.After three months of treatment,the clinical efficacy of the two groups was evaluated,and the clinical spasticity index(CSI),fine motor development motor scale-fine motor(PDMS-FM),gross motor function(GMFM),and modified Ashworth scale(MAS)scores before and after treatment were compared.Results The clinical efficacy of the study group was better than that of the control group(P<0.05).After treatment,the scores of PDMS-FM and GMFM in the two groups were higher than those before treatment,and the study group were higher than those in the control group(P<0.05).After treatment,the upper and lower limb MAS scores in the two groups were lower than before treatment,and the study group were lower than those in the control group(P<0.05).After treatment,the scores of clonus,muscle tension,tendon reflex,and CSI in the two groups were lower than those before treatment,and the study group were lower than those in the control group(P<0.05).Conclusion Acupuncture and massage along meridians has a better therapeutic effect on children with spastic cerebral palsy,which can effectively alleviate the muscle spasticity and improve the motor function of children.
