摘　　要：目的探讨髌骨爪联合髌骨针张力带治疗髌骨粉碎性骨折的疗效。方法采用髌骨爪联合髌骨针张力带治疗64例髌骨粉碎性骨折患者。记录术后骨折愈合情况、膝关节活动度以及并发症发生情况。采用Lysholm膝关节功能评分标准评价疗效。结果患者均获得随访,时间8~22个月。术后1例发生切口感染,其余切口均一期愈合。术后摄X线片复查显示,骨折均对位良好,关节面平整,4~8个月骨折均愈合,无内固定物松动、断裂等情况发生。末次随访时,63例膝关节伸直0°、屈曲120°~145°,1例伸直0°、屈曲90°;根据Lysholm膝关节功能评分标准评定疗效:优44例,良16例,可2例,差2例,优良率93.75%。结论髌骨爪联合髌骨针张力带是治疗髌骨粉碎性骨折的有效方法,具有固定牢靠、患者能早期功能锻炼、并发症少等优点。Objective To investigate the effect of patellar claw combined with patellar needle tension band on comminuted fracture of the patella.Methods The 64 patients with comminuted fractures of patella were treated by patellar claw combined with patellar needle tension band.Fracture healing,knee range of motion and complications were recorded.Lysholm knee function score was used to evaluate the therapeutic effect.Results All patients were followed up for 8~22 months.Postoperative incision infection occurred in 1 case,and the other incisions healed in one-stage.Postoperative X-ray reexamination showed that the fracture alignment was good,the joint surface was flat,and all fractures healed within 4~8 months,without any loosening or fracture of internal fixation.At the last follow-up,63 patients had knee extension of 0°and flexion of 120°~145°.One case was extended 0°and flexed 90°.The efficacy was evaluated according to Lysholm knee function score:excellent in 44 cases,good in 16 cases,fair in 2 cases,poor in 2 cases,with an excellent-good rate of 93.75%.Conclusions Patellar claw combined with patellar needle tension band is an effective method for the treatment of comminuted patellar fracture,which have the advantages of firm fixation,early functional exercise and few complications.
