肘内侧小切口辅助闭合复位经皮穿针术治疗儿童伸直桡偏型难复性肱骨髁上骨折的效果分析

摘　　要：目的:探讨肘内侧小切口辅助闭合复位经皮穿针术治疗儿童伸直桡偏型难复性肱骨髁上骨折的效果。方法:回顾性分析周口骨科医院收治的41例伸直桡偏型难复性骨折患儿的临床资料,其中于2017年9月—2022年6月行肘内侧小切口辅助闭合复位经皮穿针术治疗的患儿24例作为试验组,2009年1月—2015年12月行肘外侧入路切开复位内固定手术治疗的患儿17例作为对照组。比较两组关节功能及手术相关指标。结果:试验组关节功能优良率高于对照组,差异有统计学意义(P<0.05)。试验组手术时间、骨折愈合时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.01)。结论:肘内侧小切口辅助闭合复位经皮穿针术治疗儿童伸直桡偏型难复性肱骨髁上骨折的效果显著,能促进患儿术后恢复,缩短手术时间,减少术中出血量。Objective:Investigate the effect of small medial elbow incision assisted closed reduction and percutaneous puncture in the treatment of children's extended radial deviation type irreducible supracondylar fracture of humerus.Methods:The clinical data of 41 children with extended radial deviation type irreducible fracture admitted to Zhoukou Orthopedics Hospital were retrospectively analyzed,including 24 children treated with small medial elbow incision assisted closed reduction and percutaneous puncture from September 2017 to June 2022 as the experimental group,and 17 children treated with open reduction and internal fixation surgery through lateral elbow approach from January 2009 to December 2015 as the control group.The joint function and surgery-related indexes in two groups were compared.Results:The excellent rate of joint function in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).The operation time and fracture healing time in the experimental group were shorter than those in the control group,and the intraoperative blood loss in the experimental group was less than that in the control group,and the difference was statistically significant(P<0.01).Conclusion:Small medial elbow incision assisted closed reduction and percutaneous puncture has a significant effect in the treatment of children's extended radial deviation type irreducible supracondylar fracture of humerus,which can promote the postoperative recovery,shorten the operation time and reduce the intraoperative blood loss.
