摘　　要：目的:探讨不同程度肺动脉高压尘肺病患者球形指数(SI)及心功能的差异性。方法:选取2022年1月—2023年1月湖南省职业病防治院收治的尘肺病患者107例作为观察组,根据肺动脉高压严重程度分为排除组(n=43)、中度组(n=21)、高度组(n=43),再选取同期健康体检者45例作为对照组。比较四组SI、右房左右径、右心室面积变化率(FAC)、三尖瓣环收缩期位移(TAPSE)。结果:四组SI比较,高度组<中度组<排除组<对照组,差异有统计学意义(P<0.05)。高度组右室右房左右径、FAC、TAPSE小于对照组,差异有统计学意义(P<0.05)。结论:肺动脉高压尘肺病患者随着肺动脉压升高,SI降低,右心功能下降,右房左右径虽减小但仍在正常范围。Objective:To explore the differences in spherical index(SI)and cardiac function in pneumoconiosis patients with different degrees of pulmonary hypertension.Methods:A total of 107 patients with pneumoconiosis who were admitted to Hunan Prevention and Treatment Institute for Occupational Diseases from January 2022 to January 2023 were selected as the observation group.According to the severity of pulmonary hypertension,the patients were divided into exclusion group(n=43),moderate group(n=21),and high group(n=43).And 45 healthy individuals for physical examination during the same period were selected as the control group.The SI,right atrial transverse diameter,right ventricular fractional area change(FAC),and tricuspid annular plane systolic excursion(TAPSE)in four groups were compared.Results:Comparison of SI in the four groups showed the high group<the moderate group<the exclusion group<the control group,and the difference were statistically significant(P<0.05).The right ventricular and right atrial transverse diameter,FAC and TAPSE in the high group were less than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:In pneumoconiosis patients with pulmonary hypertension,along with the increasing pulmonary artery pressure,SI decreases,the right heart function decreases,and the right atrial transverse diameter decreases but is still within the normal range.
