炔雌醇环丙孕酮片联合坤泰胶囊治疗围绝经期功能失调性子宫出血43例疗效观察

摘　　要：目的探析围绝经期功能失调性子宫出血(功血)患者行炔雌醇环丙孕酮片联合坤泰胶囊的应用效果,观察其对血管内皮生长因子及子宫内膜厚度的影响。方法采用前瞻性随机对照研究方法,纳入南昌市洪都中医院2019年10月至2022年10月接诊围绝经期功血患者86例,随机数字表法分为对照组(n=43)和观察组(n=43)。对照组给予炔雌醇环丙孕酮片治疗,观察组加用坤泰胶囊,比较两组临床疗效,血管内皮生长因子[血管内皮生长因子(VEGF)与碱性成纤维细胞生长因子(bFGF)],子宫内膜厚度以及不良反应。结果观察组总有效率高于对照组(P<0.05)。治疗3个疗程后,两组VEGF、bFGF高于治疗前,且观察组较对照组高(P<0.05);患者子宫内膜厚度较治疗前低,观察组低于对照组(P<0.05);两组不良反应发生情况比较,差异无统计学意义(P>0.05)。结论围绝经期功血患者行炔雌醇环丙孕酮片联合坤泰胶囊后疗效显著,能提高血管内皮生长因子水平,下调子宫内膜厚度。Objective To investigate the effect of ethinyl estradiol cyproterone tablets combined with kuntai capsules in patients with dysfunctional uterine bleeding during perimenopause,observe its effect on vascular endothelial growth factor and endometrial thickness.Methods A prospective study was conducted on 86 patients with perimenopausal dysfunctional uterine bleeding admitted to Nanchang Hongdu Traditional Chinese Medicine Hospital from October 2019 to October 2022.They were randomly divided into a control group(n=43)and an observation group(n=43)using the random number table method.The control group was treated with ethinylestradiol cyproterone tablets,and the observation group was treated with kuntai capsules on the basis of the control group.The clinical efficacy criteria,vascular endothelial growth factor[vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF)],endometrial thickness and adverse reactions were compared.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After 3 cycles of treatment,the levels of VEGF and bFGF in both groups were higher than before treatment,the observation group was higher than the control group(P<0.05);the thickness of the patient's endometrium was lower than before treatment,the observation group was lower than the control group(P>0.05);there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The effect of ethinylestradiol cyproterone tablets combined with kuntai capsules on perimenopausal dysfunctional uterine bleeding patients is significant,which can increase the level of vascular endothelial growth and reduce the thickness of endometrium.
