膝关节退行性骨性关节炎中医舌诊的探讨

摘　　要：随着我国人口老龄化日益加重,作为多发性老年骨病之一的膝关节退行性骨性关节炎也越来越受到重视。中医舌诊是中医骨病四诊合参的重要环节,但大多舌诊图谱对于骨科疾病缺乏特异性,这导致部分低年资中医骨科临床医师对于中医骨病的舌诊未能有一个明确直观的认识。本研究按照膝关节退行性骨性关节炎中西医诊断标准,收录襄阳市中医医院2021年-2022年诊治的膝关节退行性骨性关节炎患者126例,通过纳入标准和排除标准筛选出其中50例,男、女各25例,年龄40~70岁,病程0.5~15年。再经由工作经验10年以上的一名副主任医师及两名主治医师商量讨论,依照临床经验与现行舌诊标准筛选出该疾病常见分型的典型舌象。结果提示患者中医舌诊与其中医临床分型的相关诊断关系密切,一定程度上反映了膝关节退行性骨性关节炎中医常见临床分型中舌象表现。中医舌诊对临床中医证候分型具有明确的诊断意义和重要价值,可通过该疾病舌诊的探讨为部分低年资中医骨科临床医师提供舌象的直观认识,为临床医生随后的临床诊疗提供一些借鉴参考,便于充分的发挥传统中医的优势,进一步提高临床疗效,为中医骨科其他相关疾病舌象的进一步研究奠定基础。With the increasing aging of Chinese population,knee degenerative osteoarthritis,as one of the multiple senile bone diseases,has been paid more and more attention.Tongue diagnosis is an important link of the four diagnoses of bone diseases in TCM.However,most of the map of tongue diagnosis lacks specificity for orthopedic diseases,which leads to the failure of some junior orthopedic clinicians to have a clear and intuitive understanding of tongue diagnosis of bone diseases in TCM.In this study,according to the diagnostic criteria of TCM and Western medicine for degenerative osteoarthritis of the knee,126 patients with degenerative osteoarthritis of the knee treated in Xiangyang Hospital of Traditional Chinese Medicine from 2021 to 2022 were included.Among them,50 cases were screened out by following the inclusion criteria and exclusion criteria,with 25 males and 25 females,aged 40 to 70 years old,and the course of disease was half a year to 15 years.After discussion by one deputy chief physician and two attending physicians with more than 10 years of working experience,typical tongue manifestations of the common classification of the disease were screened according to clinical experience and current tongue diagnostic criteria.There is a close relationship between TCM tongue diagnosis and TCM clinical classification of patients,which reflects the tongue manifestations in the common TCM clinical classification of knee degenerative osteoarthritis to a certain extent.TCM tongue diagnosis has a clear diagnostic significance and important value for clinical TCM syndrome classification.Through the discussion of tongue diagnosis of this disease,it can provide intuitive understanding of tongue image for some orthopedic clinicians of TCM with junior experience,and provide some references for clinicians in subsequent clinical diagnosis and treatment,so as to give full play to the advantages of TCM and further improve its clinical efficacy.It lays a foundation for further research on tongue image of other related diseases in
