冶尕西运用火针治疗顽固性口腔溃疡经验

摘　　要：顽固性口腔溃疡是指常发于唇内侧、舌、颊内、软腭等角化层薄或角化差的黏膜处的不同规格的溃疡,疼痛剧烈,影响饮食和说话,经系统治疗疼痛等临床症状仍然存在,为一种慢性口腔黏膜病。该病是目前临床上最常见的慢性口腔黏膜病,任何季节、不论性别、任何年龄均可发生,发病原因多样,发病机制目前尚无明确解释。该病疼痛难耐、缠绵难愈,严重降低患者生活质量。目前西医治疗主要予以消炎、止痛药物来减轻临床症状,治疗效果不一。中医治疗主要予以中药汤剂清热解毒,见效慢,疗程较长。冶尕西主任通过多年临床与教学工作实践经验主张顽固性口腔溃疡病性虚实夹杂,病机为火热之邪熏灼口舌,其证型以心脾热盛、瘀热内结、阴虚火旺为主。冶尕西主任使用火针治疗本病主要从以热治热、补虚泻实的角度考虑。火针具有针刺及灸法的双重功效,既有针刺疏经通络、调整脏腑阴阳的功能,也有灸法温经散寒、行气扶阳散结的特点,能行气逐瘀、散毒祛腐,又能温经活血、祛瘀化新。现代研究证明,火针的热力作用既可激活免疫系统,使得炎症局限化,又能改善病变局部充血、缺血等病理变化。火针治疗顽固性口腔溃疡成本低廉,起效迅速,效果喜人。Refractory oral ulcer is a chronic oral mucosal disease with severe pain that affects diet and speech.It is usually found in the mucosa with thin or poor keratosis on the inside of the lip,tongue,buccal and soft palate.It is a chronic oral mucosal disease in which pain and other clinical symptoms persist after systematic treatment.This disease is the most common chronic oral mucosal disease in clinical practice.It can occur in any season,regardless of gender and age.The causes of the disease are various,and the pathogenesis has not been clearly explained.The disease is painful,lingering and difficult to heal,which seriously reduces the quality of life of patients.At present,Western medicine mainly uses anti-inflammatory and analgesic drugs to reduce clinical symptoms,but the therapeutic effect is mixed.Traditional Chinese medicine is mainly used to clear heat and detoxify,the effect is slow and the course of treatment is long.Through many years of clinical practice and teaching experience,Director YE Gaxi holds that refractory oral ulcer is mixed with deficiency and excess,the pathogenesis of which is the burning of the mouth and tongue by the evil heat,and the syndrome types are mainly heat flourishing of the heart and spleen,heat and stasis internal staganation,Yin(阴)deficiency and fire flourishing.Therefore,Chief Director YE Gaxi applies fire needle in the treatment of this disease from the perspective of heat treatment,reinforcing deficiency and reducing excess.Fire acupuncture has the dual effects of acupuncture and moxibustion.It not only has the functions of acupuncture to dissipate the meridians and dredge collaterals,and adjust Yin and Yang(阳)of Zang-fu organs,but also has the characteristics of moxibustion to warm the meridians and disperse the cold,promote Qi(气)and support Yang.It can expel blood stasis,dissipate poison and remove the rotten,warm the meridian and promote blood circulation,remove blood stasis and generate the new.Modern studies have proved that the thermal effect of fire needle ca
