益气养阴方治疗气阴两虚型干眼的临床研究

摘　　要：目的:观察评价益气养阴方治疗气阴两虚型干眼的临床疗效。方法:将76例(152眼)符合纳入标准的干眼患者依照完全随机原则分为两组,治疗组和对照组各38例(76眼)。对照组给予0.1%玻璃酸钠滴眼液滴眼,治疗组在对照组的基础上给予中药益气养阴方加减口服,疗程均为2个月。观察比较治疗前后两组患者的眼部症状积分、泪液分泌试验、泪膜破裂时间(Break Up Time,BUT)、角膜荧光染色等监测指标的变化,以及治疗后两组间的疗效差异。结果:治疗后,两组眼部症状积分、角膜荧光染色与治疗前比较差异有统计学意义(P<0.01),治疗组泪液分泌量、BUT与治疗前比较差异也有统计学意义(P<0.01);其中,治疗组的眼干涩、疲劳感、异物感、畏光积分项疗效差异有统计学意义(P<0.05),烧灼感积分P=0.44;对照组眼干涩、疲劳感、异物感积分项治疗前后差异有统计学意义(P<0.05),而烧灼感、畏光积分疗效差异无统计学意义(P>0.05)。两组患者均较治疗前改善,且治疗组明显优于对照组。结论:益气养阴方治疗气阴两虚型干眼疗效肯定,其有效缓解了患者的各种症状,可增加泪液分泌量,延长BUT,改善角膜上皮破损情况,是一种切实有效的治疗方法。Objective:To observe and evaluate the clinical effects of the therapeutic of“Yiqi Yangyin decoction(益气养阴方)”in the treatment of dry eyes with deficiency of both Qi(气)and Yin(阴).Methods:A total of 76 patients(152 eyes)with dry eyes were selected and randomly divided into the treatment group and control group,38 cases(76 eyes)in each group.The control group was treated with 0.1%sodium hyaluronate eye drops.The treatment group was treated with Yiqi Yangyin decoction more.The treatment course was 2 months.The changes of eye symptom score,tear secretion,BUT,corneal fluorescence staining and other monitoring indicators between the two groups before and after treatment were observed and compared,and the efficacy between the two groups after treatment was compared.Results:After treatment,the results of ocular symptom score and corneal fluorescence staining in both groups were significantly improved(P<0.01),while there were also significant improvement in tear secretion and BUT in the treatment group(P<0.01).Among them,the difference in efficacy of eye dryness,eye strain,foreign body sensation,and photophobia in the treatment group was statistically significant(P<0.05),and P=0.44 with the burning sensation score.The differences in efficacy of eye dryness,eye strain,foreign body sensation in the control group was statistically significant(P<0.05)as well,while there was no statistically significant difference in the burning sensation and photophobia(P>0.05).The patients in both groups were better than that before treatment,and the improvement in the treatment group was superior to the control group(P<0.01).Conclusion“:Yiqi Yangyin decoction”can effectively relieve various symptoms caused by dry eye,increase tear secretion,prolong break-up time and ameliorate corneal epithelial damage,which is an effective and feasible therapy.
