扶正解毒法在支气管肺癌治疗中的运用

摘　　要：支气管肺癌(以下简称肺癌)是临床常见的呼吸道恶性肿瘤,其发病率及致死率在所有癌种中均位居前列。中医药治疗肺癌从辨证论治出发。《黄帝内经》:“正气存内,邪不可干。”正气亏虚,致机体感受六淫邪毒、肺脏功能失调、气血津液运行失畅,继而在体内形成血瘀、气滞、痰凝等病理产物,日久相互胶结形成癌毒。故笔者导师认为正气亏虚、癌毒内生是肺癌致病的关键。且现代医学治疗(包括手术治疗、化学治疗、放射治疗、靶向治疗等)具有耗伤气血、损伤正气之势,故临床以扶正解毒为治则,扶正当以固护脾胃为原则,以补中益气、健脾助运、兼以祛湿为治法;解毒当以化痰散结、活血祛瘀、清热祛邪为原则,以理气化痰、软坚散结、活血化瘀、清热解毒为治法。导师在此理论基础上结合药理研究,创立治疗肺癌经验方——金康化积方,临床应用中以金康化积方为基础,扶正解毒贯穿始终,并把握祛邪和扶正的关系,使祛邪而不伤正,扶正而不恋邪,灵活选择用药,根据患者体质及疾病分期各有侧重,根据患者的症状变化而加减。文章结合1则临床案例,总结导师治疗肺癌临床经验,在遣方过程中展现治疗思路,以期继承和发扬扶正解毒法治疗肺癌的学术思想。Bronchial lung cancer(hereinafter referred to as lung cancer)is a common clinical malignant tumor of the respiratory tract.Its incidence rate and mortality rate are at the forefront of all cancers.The treatment of lung cancer in traditional Chinese medicine starts from syndrome differentiation and treatment.Huangdi Neijing(《黄帝内经》)recorded:The evil cannot interfer when there is enough healthy Qi(气).Deficiency of healthy Qi can cause the body to experience the six evils and toxins,lung dysfunction,and impaired circulation of Qi,blood,and body fluids,causing the formation of pathological products such as blood stasis,Qi stagnation,and phlegm coagulation in the body,which finally forms cancer toxins.Therefore,the supervisor believes that the deficiency of healthy Qi and the endogenesis of cancer toxin are the key factors in the pathogenesis of lung cancer.Moreover,modern medical treatment(including surgical treatment,chemotherapy,radiation therapy,targeted therapy,etc.)has the potential of consuming Qi and blood,and damaging the healthy Qi.Therefore,in clinical practice,the principle of strengthening the spleen and stomach should be based on the principle of nourishing the middle and supplementing Qi,strengthening the spleen and promoting circulation,and also removing dampness.Detoxification should be based on the principles of resolving phlegm and resolving stasis,promoting blood circulation and removing blood stasis,clearing heat and removing pathogenic factors,and the treatment should be based on regulating Qi and resolving phlegm,softening hardness and resolving stasis,promoting blood circulation and resolving blood stasis,and clearing heat and detoxifying.On the basis of this theory and combined with pharmacological research,the author’s supervisor establishes an empirical formula for the treatment of lung cancer-the Jinkang Huaji decoction(金康化积方).In clinical application,the jinkang Huaji decoction is used as the basis,and the relationship between dispelling pathogenic factors and promo
