上焦宣痹汤在新型冠状病毒感染后遗症中的应用

摘　　要：上焦宣痹汤是叶天士变通栀子豉汤,寓“微苦微辛”法治疗湿热痹阻上焦的代表方之一,立方特点在于舒气以透湿兼宣达上焦。方中所选诸药平淡轻灵,寓“轻可去实”之意。新型冠状病毒奥密克戎的变异株有着较强的传播能力与免疫逃逸能力,属于中医“疫”的范畴。新冠病毒感染后遗症期的患者,在发病初期,服用退热药物后多见大汗出,伤及阴液,余邪未尽,正气渐损,加之湿邪黏滞,易阻气机,故疾病缠绵难愈,湿、热、郁是本病的主要特点。在治疗新冠病毒感染后遗症的上焦相关病症时,应以“轻清透湿舒气”为要,可选用上焦宣痹汤为基本方加减治之。Shangjiao Xuanbi Decoction is a modified version of Zhizichi Decoction,which was created by Ye Tianshi as a representative formula for treating Damp-Heat obstruction in the Shangjiao,by using the method of using"slightly bitter and slightly pungent"drugs.Its unique feature lies in its ability to promote Qi circulation,resolve dampness,and invigorate the Shangjiao.The selected herbs in this formula are mild and gentle,with the intention of"being gentle yet effective in removing excess".The variant strains of the novel corona virus,such as Omicron,have strong transmission and immune evasion capabilities,falling into the category of"epidemic"in traditional Chinese medicine.Patients in the sequelae stage of COVID-19 infection often experience profuse sweating after taking antipyretic drugs in the early stage of the disease,which damages the Yin Fluid and weakens the righteous Qi.With combination of Dampness and Qi stagnation,it easily obstructs the flow of Qi,leading to a lingering and difficult-to-cure illness.Dampness,Heat,and Qi stagnation are the main characteristics of this disease.When treating the Shangjiao-related symptoms in the sequelae of COVID-19 infection,the principle of"being gentle,clear,promoting Dampness and invigorating Qi"should be followed.Therefore,Shangjiao Xuanbi Decoction can be used as the basic formula with modifications.
