基于少阳三焦理论辨治三焦郁阻型前庭性偏头痛

摘　　要：风、痰、热、瘀是前庭性偏头痛急性发作的主要病理因素,与少阳、三焦枢机不畅密不可分。少阳枢机不畅、三焦郁阻是前庭性偏头痛的主要病机。张怀亮教授基于少阳三焦理论,综合运用脏腑辨证、气血津液辨证、经络辨证,创新了三焦辨治体系,设立了疏利少阳、三焦气机的基本治则,根据症状特点分期辨治,依据气、火、津液异常侧重不同,以柴芩温胆汤为基础方加减治前庭性偏头痛,在临床中取得了较好的疗效。基于异病同治理论,其他疾病引起的眩晕如后循环缺血性眩晕、良性阵发性位置性眩晕、前庭神经炎等以及郁证亦可从少阳三焦辨证施治,收效明显。Wind,Phlegm,Heat and blood stasis are the main pathological factors of acute attack of vestibular migraine,which are inseparable from the Qi stagnation of Shaoyang and Triple Energizer.The main pathogenesis of vestibular migraine is Shaoyang axis Qi obstruction and Triple Energizer Qi Stagnation.Professor Zhang Huailiang,based on the theory of Shaoyang and Triple Energizer,comprehensively applied the syndrome differentiation of Zang-Fu organs,Qi and blood as well as body Fluid and meridians,innovated the syndrome differentiation and treatment system of Triple Energizer,and established the basic treatment principles of dredging Qi movement of Shaoyang and Triple Energizer,differentiated and treated it by stages according to the symptoms and characteristics,and focused on different abnormalities of Qi,Fire and body Fluid.Modified Chaiqin Wendan Decoction(Xuan Da Drink)was used as the basis formula to treat vestibular migraine,and good clinical effects is achieved.Based on the theory of treating different diseases with the same method,other kinds of vertigo caused by different diseases,such as posterior circulation ischemic vertigo,benign paroxysmal positional vertigo,vestibular neuritis,etc.,and depression syndrome can also be treated based on syndrome differentiation of Shaoyang and Triple Energizer,with obvious clinical efficacy.
