张怀亮运用熄风止晕合剂治疗良性阵发性位置性眩晕复位后残余头晕的经验

摘　　要：张怀亮根据多年临证经验,结合《黄帝内经》《难经》中关于三焦的论述以及《伤寒论》中“少阳之为病,口苦,咽干,目眩”的少阳病提纲,发现少阳三焦枢机失运与良性阵发性位置性眩晕复位后残余头晕的发生有密切联系,认为本病的发生应责之三焦郁阻,提出了三焦郁阻致晕的观点:即眩晕的发生与风、痰、瘀、热关系密切,而风、痰、瘀、热的产生源于三焦郁阻。张怀亮临证时从少阳三焦入手,通过疏利三焦水火气血治疗该病,探索出治疗风痰上扰所致眩晕的经验方——熄风止晕汤,并创制熄风止晕合剂,临床可有效缓解良性阵发性位置性眩晕复位后残余头晕。此外,但凡因少阳郁遏、风痰上扰而导致的头晕,不论西医病因为何,均可应用熄风止晕合剂治疗。Based on many years'clinical experience,Zhang Huailiang,in combination with the discussion on Triple Energizer in Huang Di Nei Jing and Nan Jing as well as the illustration that"the symptoms of Shaoyang disease aare bitter taste,dry throat and dizziness"in Treatise on Febrile Diseases,found that the cardinal dysfunction of Triple Energizer was closely related to the occurrence of residual dizziness after the restoration of benign paroxysmal positional vertigo,and he proposed that obstruction in Triple Energizer should be responsible for the occurrence of the disease.In other words,the occurrence of vertigo is closely related to Wind,Phlegm,blood stasis and Heat,all of which comes from the stagnation in Triple Energizer.Zhang Huailiang started with Shaoyang Triple Energizer in clinical practice,and treated the disease by dredging the Fire,Water,Qi and blood of Sanjiao,and explored the formula of treating dizziness caused by Wind and Phlegm disturbance,i.e.Xifeng Zhiyun Decoction,and created Xifeng Zhiyun Mixture,which can effectively relieve the residual dizziness after the restoration of benign paroxysmal positional vertigo.In addition,all kinds of dizziness caused by stagnation of Shaoyang and disturbance of Wind and Phlegm,regardless of the cause from perspective of western medicine,can be treated with Xifeng Zhiyun Mixture and stopping dizziness,with good clinical efficacy.
