椅背屈肘复位法联合舒筋活血汤加减治疗肩关节前脱位临床研究

摘　　要：目的:观察椅背屈肘复位法联合舒筋活血汤加减治疗肩关节前脱位的临床疗效。方法:采用随机数字表法将60例肩关节前脱位患者分为观察组、对照组各30例。对照组采用舒筋活血汤加减联合手牵足蹬法治疗,观察组采用椅背屈肘复位法联合舒筋活血汤加减治疗。比较2组治疗前后视觉模拟评分法(VAS)评分、Constant-Murley肩关节功能评分;比较2组肩关节功能优良率、复位时间、1次复位成功率及不良反应发生情况。结果:观察组复位时间短于对照组(P<0.05),1次复位成功率高于对照组(P<0.05)。治疗后,2组VAS评分明显低于治疗前(P<0.05),Constant-Murley肩关节功能维度(日常活动、疼痛、肌力、肩关节活动度)评分及总分均高于治疗前(P<0.05);观察组治疗后VAS评分低于对照组(P<0.05),ConstantMurley肩关节功能评分高于对照组(P<0.05)。观察组肩关节功能优良率96.67%,高于对照组80.00%(P<0.05)。2组均未发生不良反应。结论:椅背屈肘复位法联合舒筋活血汤加减治疗肩关节前脱位疗效确切,能够缩短患者复位时间,改善患者疼痛及肩关节功能,安全性较高。Objective:To observe the clinical effect of the therapy of the chair reduction method combined with modified Shujin Huoxue Decoction on anterior shoulder dislocation.Methods:A total of 60 patients with anterior shoulder dislocation were divided into the observation group and the control group according to random number table method,with 30 cases in each group.The control group was treated with modified Shujin Huoxue Decoction combined with arms-holding and legs-pushing-off method,and the observation group was treated with chair reduction method combined with modified Shujin Huoxue Decoction.The scores of Visual Analogue Scale(VAS)and Constant-Murley Score were compared before and after treatment between the two groups.The excellent-and-good rate of shoulder function,the reduction time,the success rate of one reduction and the occurrence of adverse reactions were also compared between the two groups.Results:The reduction time in the observation group was shorter than that in the control group(P<0.05),and the success rate of one reduction was higher than that in the control group(P<0.05).After treatment,the scores of VAS in the two groups were lower than those before treatment (P<0.05),and the scores of each dimension of Constant-Murley Score,including daily activity,pain, muscle strength, and shoulder motion, and the total scores were higher than those beforetreatment (P<0.05);the scores of VAS in the observation group were lower than those in the control group(P<0.05), and the Constant- Murley Score was higher than that in the control group (P<0.05). Theexcellent- and- good rate of shoulder function in the observation group was 96.67%,higher than that of80.00% in the control group (P<0.05). No adverse reaction occurred in the two groups. Conclusion: Thetherapy of chair reduction method combined with modified Shujin Huoxue Decoction has a definite effect onanterior shoulder dislocation, which can shorten the reduction time of patients, improve their pain andshoulder joint function,with great safety.
