泻心保和汤加减联合西药治疗小儿湿热型亚急性湿疹临床研究

摘　　要：目的:观察泻心保和汤加减联合西药治疗小儿湿热型亚急性湿疹的临床疗效。方法:采用随机数字表法将85例湿热型亚急性湿疹患儿分为观察组(43例)与对照组(42例)。对照组给予常规西药治疗,观察组在对照组基础上给予泻心保和汤加减治疗。比较2组治疗前后中医证候积分、湿疹面积及严重度指数(EASI)、瘙痒程度、儿童皮肤病生活质量指数(CDLQI)评分;比较2组临床疗效、复发率及不良反应发生情况。结果:观察组治疗总有效率高于对照组(P<0.05)。治疗后,2组中医证候积分、EASI、瘙痒程度、CDLQI评分均较治疗前降低(P<0.05),且观察组治疗后中医证候积分、EASI、瘙痒程度、CDLQI评分均低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。随访6个月,观察组复发率低于对照组(P<0.05)。结论:泻心保和汤加减联合西药治疗湿热型亚急性湿疹疗效确切,能改善患儿皮损和瘙痒症状,提高生活质量,降低复发率,安全性较高。Objective:To observe the clinical effect of the therapy of modified Xiexin Baohe Decoction combined with western medicine on children subacute eczema of damp-heat type.Methods:A total of 85 children with subacute eczema of damp-heat type were divided into the observation group(43 cases)and the control group(42 cases)according to random number table method.The control group was treated with conventional western medicine,and the observation group was additionally treated with modified Xiexin Baohe Decoction based on the treatment of the control group.The scores of traditional Chinese medicine(TCM)syndromes,Eczema Area and Severity Index(EASI),itching degree and Children Dermatology Life Quality Index(CDLQI)were compared between the two groups before and after treatment.The clinical effect,recurrence rate and occurrence of adverse reactions were also compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of TCM syndromes,EASI,itching degree and CDLQI in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group after treatment(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After six months of follow-up,the recurrence rate in the observation group was lower than that in the control group (P<0.05). Conclusion: The therapy of modified Xiexin Baohe Decoction combined withwestern medicine has a definite effect on subacute eczema of damp- heat type, which can improve thesymptoms of skin lesions and itching,improve the quality of life,reduce the recurrence rate,and havehigh safety.
