摘 要:目的观察多糖铁复合物胶囊和琥珀酸亚铁治疗妊娠期缺铁性贫血的效果及对网织红细胞、铁代谢参数的影响。方法将妊娠期缺铁性贫血患者随机分为对照组和试验组。对照组给予琥珀酸亚铁,每次餐后服用0.2 g,每日2次;试验组给予多糖铁复合物胶囊,每次餐后服用0.15 g,每日2次;均连续服用6周。比较2组临床疗效、网织红细胞计数、铁代谢参数、药物不良反应及妊娠结局。结果试验组和对照组各48例。试验组与对照组临床总有效率分别为95.83%和85.42%,差异无统计学意义(P>0.05)。治疗后,试验组网织红细胞百分比和高、中荧光强度网织红细胞百分比分别为(2.55±0.20)%、(2.16±0.09)%、(8.45±1.85)%,对照组分别为(2.37±0.19)%、(2.49±0.10)%、(7.26±1.71)%,差异均有统计学意义(均P<0.05)。治疗后,试验组和对照组的网织红细胞绝对值计数分别为(38.87±4.06)×109·L^(-1)和(35.27±3.95)×10^(9)·L^(-1);铁蛋白(SF)分别为(32.48±6.07)和(29.35±5.45)μg·L^(-1);转铁蛋白(TRF)分别为(2.34±0.69)和(2.88±0.73)g·L^(-1);血清铁(SI)分别为(13.18±1.50)和(10.75±1.39)μmol·L^(-1),差异均有统计学意义(均P<0.05)。试验组和对照组的药物不良反应发生率分别为4.17%和16.67%,差异有统计学意义(P<0.05)。试验组和对照组不良妊娠结局发生率分别为6.25%和14.58%,差异无统计学意义(P>0.05)。结论多糖铁复合物胶囊、琥珀酸亚铁治疗妊娠期缺铁性贫血的疗效差异无统计学意义,但多糖铁复合物胶囊在改善网织红细胞、铁代谢参数方面更佳,药物不良反应发生率低。Objective To observe the effects of polysaccharide iron complex capsules and ferrous succinate on iron deficiency anemia during pregnancy and its influences on reticulocyte and iron metabolism parameters.Methods The patients with iron deficiency anemia during pregnancy were randomly divided into control group and treatment group.The control group was given ferrous succinate,0.2 g after each meal,twice a day.The treatment group was given polysaccharide iron complex capsule,0.15 g after each meal,twice a day.All patients were treatment for 6 weeks.The clinical efficacy,reticulocyte count,iron metabolism parameters,adverse drug reactions and pregnancy outcome were compared between the two groups.Results There were 48 cases in treatment group and 48 cases in control group.The total clinical effective rates of treatment group and control group were 95.83%and 85.42%,with no statistically significant differece(P>0.05).After treatment,the percentage of reticulocytes and the high,middle fluorescence intensity reticulocyte percentages were(2.55±0.20)%,(2.16±0.09)%,(8.45±1.85)%,which in control group were(2.37±0.19)%,(2.49±0.10)%,(7.26±1.71)%,the differences were statistically significant(all P<0.05).After treatment,the absolute count of reticulocytes in treatment group and control group were(38.87±4.06)×10^(9)·L^(-1) and(35.27±3.95)×10^(9)·L^(-1);ferritin(SF)were(32.48±6.07)and(29.35±5.45)μg·L^(-1);transferrin(TRF)were(2.34±0.69)and(2.88±0.73)g·L^(-1);serum iron(SI)were(13.18±1.50)and(10.75±1.39)μmol·L^(-1),the differences were statistically significant(all P<0.05).The incidences of adverse drug reactions in treatment group and control group were 4.17%and 16.67%,the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in treatment group and control group were 6.25%and 14.58%,with no significant difference(P>0.05).Conclusion There is no statistical difference between polysaccharide iron complex capsule and ferrous succinate in the treatment of iron deficiency an
