安神定志丸加减治疗心虚胆怯型心房颤动患者的临床研究

摘　　要：目的观察安神定志丸加减治疗心虚胆怯型心房颤动的临床疗效。方法选取我院2018年1月至2022年12月收治的64例心虚胆怯型心房颤动患者,根据就诊顺序进行分组,单序号的32例患者为观察组,双序号的32例患者为对照组。对照组口服达比加群酯胶囊及琥珀酸美托洛尔缓释片,观察组在对照组的基础上加用安神定志丸加减。对比两组治疗效果、不良反应及治疗前、后心功能、血浆同型半胱氨酸(Hcy)水平、炎性因子和生活质量。结果观察组治疗总有效率93.75%,显著高于对照组的71.88%(P<0.05),而两组不良反应发生率则无明显差异(P>0.05);两组治疗前心功能(LVEF、CI、CO)及Hcy水平无明显差异(P>0.05),经治疗后LVEF、CI、CO均有明显增高,Hcy水平则有明显降低,且观察组与对照组LVEF、CI、CO及Hcy水平比较有明显差异(P<0.05);两组治疗前炎性因子(hs-CRP、TNF-α、IL-6)水平无明显差异(P>0.05),经治疗后hs-CRP、TNF-α、IL-6水平均有明显降低,且观察组的3个指标均显著低于对照组(P<0.05);两组治疗前心房颤动患者生活质量量表(AFEQT)各维度得分无明显差异(P>0.05),经治疗后各维度得分均有明显升高,且观察组各维度得分明显高于对照组(P<0.05)。结论安神定志丸加减能有效提高心虚胆怯型心房颤动的治疗效果,并改善心功能,降低Hcy及炎性因子水平,提升生活质量,且无明显不良反应。Objective To observe the clinical efficacy of Anshen Dingzhi pill in the treatment of atrial fibrillation of heart deficiency and timidity.Method A total of 64 patients with atrial fibrillation with heart deficiency and timid type admitted to our Hospital of Traditional Chinese Medicine from January 2018 to December 2022 were selected and grouped according to the order of treatment.32 patients with single serial number were included in the observation group and 32 patients with double serial number were included in the control group.The control group was given dabigatran etexilate capsules and metoprolol succinate sustained-release tablets,and the observation group was given Anshen Dingzhi pill on the basis of the control group.The therapeutic effect,adverse reactions,cardiac function,plasma homocysteine(Hcy)level,inflammatory factors and quality of life of the two groups were compared before and after treatment.Result The total effective rate of observation group was 93.75%,which was significantly higher than that of control group(71.88%)(P<0.05),but there was no significant difference in the incidence of adverse reactions(P>0.05).There were no significant differences in cardiac function(LVEF,CI,CO)and Hcy levels between the two groups before treatment(P>0.05).After treatment,LVEF,CI,CO and Hcy levels were significantly increased,while Hcy levels were significantly decreased.There were significant differences in LVEF,CI,CO and Hcy levels between the observation group and the control group(P<0.05).There were no significant differences in the levels of inflammatory factors(hs-CRP,TNF-αand IL-6)between the two groups before treatment(P>0.05),but the levels of hs-CRP,TNF-αand IL-6 were significantly decreased after treatment,and the three indexes in the observation group were significantly lower than those in the control group(P<0.05).There were no significant differences in the scores of quality of life(AFEQT)between the two groups before treatment(P>0.05),but the scores of AFeQT were significantly increased afte
