摘 要:目的探究生物素对免疫检测的干扰及大剂量服用生物素对免疫指标的影响。方法招募哈尔滨医科大学附属第一医院体检健康的志愿者34名,并将其分为5组:对照组(10例)、低剂量组(6例)、中剂量组(6例)、高剂量组(6例)和超高剂量组(6例)。同期选择本院10例慢性心衰稳定期随访者作为稳定心衰组,采用化学发光法自动化检测平台检测心肌肌钙蛋白I、肌钙蛋白T水平,酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA )方法检测生物素水平。结果与对照组相比,稳定心衰组患者第1日至第5日生物素浓度明显降低,肌钙蛋白I、肌钙蛋白T水平明显升高,差异具有统计学意义(t1d=11.40、13.33、63.60,t2d=17.11、6.67、63.42,t3d=14.10、13.33、64.71,t4d=16.67、6.67、62.43,t5d=15.80、13.33、66.05,P值均<0.001)。与用药前相比,服用5 mg生物素后低剂量组、服用10 mg生物素后中剂量组、服用20 mg生物素后高剂量组、服用50 mg生物素超高剂量组肌钙蛋白I、肌钙蛋白T及生物素浓度均明显升高,组间差异均具有统计学意义(F=148.11、53.08、38.39、147.26、57.47、42.63、152.82、69.26、58.63、152.68、62.57、48.27,P值均<0.001)。结论生物素会干扰链霉亲和素捕获目标分析物的能力,从而导致检测结果的假性升高或者降低,影响临床判断。Objective To explore the interference of biotin on immune testing and the impact of high-dose administration of biotin on immune indicators.Methods A total of 34 healthy volunteers from the First Affiliated Hospital of Harbin Medical University were recruited and divided into five groups:control group(10 cases),low-dose group(6 cases),medium dose group(6 cases),high-dose group(6 cases)and ultra-high dose group(6 cases).At the same time,10 patients with chronic heart failure in the stable period were selected as the stable heart failure group.The levels of cardiac troponin I and troponin T were detected by chemiluminescence automated detection platform,and the levels of biotin were detected by enzyme-linked immunosorbent assay(ELISA)method.Results Compared with the control group,the concentration of biotin in the stable heart failure group was significantly lower from the first day to the fifth day,and the levels of troponin I and troponin T were significantly higher(t1d=11.40,13.33,63.60,t2d=17.11,6.67,63.42,t3d=14.10,13.33,64.71,t4d=16.67,6.67,62.43,t5d=15.80,13.33,66.05,all P values<0.001).Compared with that before treatment,the concentrations of troponin I,troponin T and biotin were all increased significantly in the low-dose group after taking of 5 mg biotin,the middle dose group after taking of 10 mg biotin,the high-dose group after taking of 20 mg biotin,and the ultra-high dose group after taking of 50 mg biotin(F=148.11,53.08,38.39,147.26,57.47,42.63,152.82,69.26,58.63,152.68,62.57,48.27,all P values<0.001).Conclusions Biotin can interfere with the ability of streptavidin to capture target analytes,leading to a false increase or decrease in detection results,affecting clinical judgment.
