摘　　要：目的研究精子DNA碎片指数(DFI)、精子高DNA着色性(HDS)与精子运动参数的相关性,探讨其对精子运动能力和运动方式影响。方法本研究为回顾性病例研究,选取2020年9—12月因健康体检、婚前、孕前、不育原因和辅助生殖前检查等需求于北部战区总医院生殖医学科男科门诊就诊的1772名纳入者,年龄(32.72±4.54)岁,年龄范围为20~50岁。分析纳入者的DFI、HDS与禁欲天数、年龄、精子前向率、活率、曲线速度、直线速度、平均速度、线性指数、直性指数及振动指数的相关性。结果精子DFI水平与禁欲天数、精子HDS水平及年龄呈正相关性(P<0.05),与前向率、活率、曲线速度、直线速度、平均速度及直性指数呈负相关性(P<0.05),与线性指数及振动指数无显著相关性(P>0.05)。精子HDS水平与精子DFI水平呈正相关性(P<0.05),与前向率、活率、曲线速度、直线速度、平均速度、线性指数及直性指数呈负相关性(P<0.05),与禁欲天数、年龄及振动指数无显著相关性(P>0.05)。结论精子DFI、HDS与精子运动参数具有相关性且不相互独立是精子运动能力和运动方式改变的重要原因,从而影响自然受精和辅助生殖妊娠结局。Objective To investigate the correlation between sperm DNA fragment index(DFI),sperm high DNA staining(HDS)and sperm motility parameters,and to explore their effects on sperm motility and motility.Methods The sperm DNA integrity and semen routine test results of 1772 patients in the Male Outpatient of Reproductive Medicine department of General Hospital of Northern Theater Command,from September to December 2020 were analyzed,the average age of the patients was(32.72±4.54)years old,and the age range was 20 to 50 years old.The correlation of sperm DNA fragment index(DFI)and high DNA staining(HDS)with abstinence days,age,progressive sperm rate,total sperm viability,curvilinear velocity,straight-line velocity,average path velocity,linearity,straightness and wobble were analyzed.Results Sperm DFI level was positively correlated with abstinence days,sperm HDS level and age,and negatively correlated with progressive sperm rate,total sperm viability,curvilinear velocity,straight-line velocity,average path velocity and straightness(P<0.05),but not with linearity and wobble(P>0.05).Sperm HDS level was positively correlated with sperm DFI level,and negatively correlated with progressive sperm rate,total sperm viability,curvilinear velocity,straight-line velocity,average path velocity,linearity and straightness(P<0.05),but not with abstinence days,age and wobble(P>0.05).Conclusions The correlation between sperm DFI,HDS and sperm motility parameters and not independent of each other is an important reason for the change of sperm motility and motility mode,which affects the outcome of natural fertilization and assisted reproductive pregnancy.
