基于中医动态思维探讨外泌体对胃癌前病变“虚瘀毒”病机的影响

摘　　要：脾虚为本、瘀毒致病是阐释胃癌前病变(GPL)发生、发展的重要理论,“虚瘀毒”病机的形成是GPL发生、癌性转化的关键。外泌体作为微环境重要组成部分,可促进病变细胞的恶性增殖、癌性改变。中医动态思维与外泌体合成、分泌有许多相同之处。基于中医动态思维,研究外泌体对GPL“虚瘀毒”病机的影响。笔者发现,“虚瘀毒”病机的形成过程符合现代医学对恶变细胞源外泌体的认识,外泌体是GPL胃黏膜局部微环境的重要生物学内容。通过深入探究外泌体与微环境成分的交互作用,希冀促进现代医学与中医学在GPL发展过程及防治策略上的融合和发展。Spleen deficiency is the foundation,blood stasis and toxin causes disease are important theories to explain the occurrence and development of gastric precancerous lesions(GPL).The formation of the microenvironment of‘deficiency,blood stasis and toxin’is the key to the occurrence and carcinomatous transformation of GPL.Exosomes are important components of microenvironment,which can promote malignant proliferation and cancerous changes of pathological cells.The dynamic thinking of traditional Chinese medicine(TCM)has many similarities with the synthesis and secretion of exosomes.Based on the dynamic thinking of TCM,the author studied the effect of exosomes on the microenvironment of GPL‘deficiency,blood stasis and toxin’.It is found that the formation of‘deficiency,blood stasis and toxin’microenvironment is in line with modern medicine’s understanding of malignant cell-derived exosomes,and exosomes are significant biological contents of‘deficiency,blood stasis and toxin’microenvironment.Through in-depth exploration of the interaction between exosomes and the microenvironmental components of‘deficiency,blood stasis and toxin’,it is hoped to promote the integration and development of modern medicine and TCM in the development process and prevention and control strategies of GPL.
