从“针足阳明,使经不传则愈”探讨针刺逆转脾瘅传变的思路

摘　　要：中医学脾瘅与糖尿病前期较为接近,张仲景《伤寒论》提出了“针足阳明,使经不传则愈”,笔者认为有助于明确针刺阻断脾瘅向消渴病传变的途径,对针刺逆转脾瘅传变具有重要的指导意义和临床价值。文章从脾瘅的病因病机着手,从阳明经病的证候表现、经脉循行特点等方面阐述了脾瘅与阳明系统病变证候特点有内在的一致性,认为脾瘅当从阳明经论治。同时分析了“针足阳明,使经不传则愈”的立论依据与具体选穴,简要综述了针刺足阳明治疗脾瘅的现代研究进展。Traditional Chinese medicine spleen illusion is close to that of pre-diabetes.ZHANG Zhongjing was put forward‘needle punching foot Yangming in preventing disease and change’in the Shanghan Lun.The authors believe that it is helpful to clarify the way that acupuncture interrupts the transmutation of the spleen illusion to thirst disease,and has important guiding significance and clinical value for reversing the transmutation of spleen illusion by acupuncture.Starting from the etiology and pathogenesis of the spleen illusion,this paper expounded the intrinsic consistency between the spleen illusion and syndrome characteristics of Yangming syndrome from the aspects of symptoms and meridional circulation,and holded that the treatment of spleen illusion should be based on Yangming meridional circulation.At the same time,the paper analyzed the theoretical basis and specific points of‘needle punching foot Yangming in preventing disease and change’,and briefly summarized the modern research progress of acupuncture‘foot Yangming’in the treatment of spleen illusion.
