摘　　要：目的基于男性勃起功能障碍(erectile dysfunction,ED)患者阴茎血流、夜间勃起功能检测结果,分析患者血清胰岛素样生长因子Ⅰ(insulin-like growth factor-I,IGF-1)水平差异及对勃起功能的影响。方法对本院就诊的203例ED患者进行阴茎海绵体注射血管活性药物试验(intracavernous injection,ICI)联合阴茎彩色多普勒超声检查(color doppler duplex ultrasonography,CDDU),将患者分为阴茎血供正常与异常两组。使用Rigiscan仪器对患者进行夜间阴茎勃起功能检测(nocturnal penile tumescence,NPT),将患者分为夜间勃起正常与异常两组。检测所有患者血清IGF-1水平,并比较阴茎血供正常与异常组、夜间勃起正常与异常组间IGF-1水平差异。最后,将患者血清IGF-1水平与阴茎供血和夜间勃起指标进行相关性分析。结果(1)阴茎血供正常组血清IGF-1水平(134.87±31.97)ng/mL高于阴茎血供异常组(122.11±39.85)ng/mL(P<0.05);(2)夜间勃起正常组血清IGF-1水平(133.30±41.69)ng/mL高于夜间勃起异常组(118.92±36.35)ng/mL(P<0.05);(3)患者血清IGF-1水平与阴茎双侧动脉最大收缩期血流速度(peak systolic velocity,PSV)(r=0.24,P<0.01)、总平均硬度(r=0.22,P<0.01)及勃起次数(r=0.25,P<0.01)均呈正相关。结论ED患者血清IGF-1水平与其阴茎供血和夜间勃起相关,有助于辅助临床ED症状的鉴别与评估。Objective Based on the results of penile blood flow and nocturnal erectile dysfunction in male erectile dysfunction(ED)patient,the differences in serum insulin-like growth factor I(IGF-1)level and the effects on erectile function were analyzed.Methods 203 patients with ED underwent the intracavernous injection(ICI)combined with color doppler duplex ultrasound(CDDU)and were divided into two groups with normal and abnormal penile blood flow.RigiScan instrument was used to evaluate the nocturnal erectile function(NPT)of the patients,and the patients were divided into two groups with normal and abnormal nocturnal erectile function.The serum IGF-1 levels of all patients were detected in order to compare the differences of ICF-1 levels between normal and abnormal groups.Finally,the correlations between serum IGF-1 level and measures of penile blood flow,nocturnal erectile function were analyzed.Results(1)The serum IGF-1 level of group with normal penile blood flow(122.11±39.85)ng/mL was significantly lower than that of group with abnormal penile blood flow(134.87±31.97)ng/mL(P<0.05).(2)The serum ICF-1 level(133.30±41.69)ng/mL of group with normal night erection was significantly higher than that of group with abnormal night erection(118.92±36.35)ng/mL(P<0.05).(3)The serum IGF-1 level was positively correlated with the peak systolic velocity(PSV)(r=0.24,P<0.01),total average rigidity(r=0.22,P<0.01)and number of erections(r=0.25,P<0.01).Conclusion The serum IGF-1 level of ED patients was related to the penile blood flow and nocturnal erection,which could assist in the identification and evaluation of ED symptoms.
