摘 要:目的通过Meta分析探究体外磁刺激对于慢性前列腺炎/慢性盆腔疼痛综合征(CP/CPPS)的干预效果。方法通过计算机检索Pubmed、EMbase、The Cochrane Library、Web of Science、CNKI和Wanfang数据库,纳入体外磁刺激干预CP/CPPS的随机对照实验研究(RCTs),文献的录用时限为2000年至2022年4月29日。由2名评价员独立筛选文献、提取资料并分析偏倚风险后,采取Revman 5.4进行Meta分析。结果最终纳入5篇研究,包括364名患有CP/CPPS的患者。Meta分析结果显示:与对照组相比,体外磁刺激可以显著改善患者的前列腺炎症状评分(NIH-CPSI)[MD=5.61;95%CI(-7.91,-3.31),P<0.00001],最大尿流率(Qmax)[MD=3.99;95%CI(2.63,5.35),P<0.00001]。勃起功能评分(IIEF-5)显著改善[MD=-0.94;95%CI(-2.08,0.21),P=0.11]。结论体外磁刺激可以显著改善CP/CPPS患者的前列腺炎症状、NIH-CPSI评分及Qmax;但目前相关临床研究数量较少,研究质量有限,尚需更多高质量临床试验进一步证明以上结论。Objective To investigate the effects of extracorporeal magnetic stimulation in the treatment of CP/CPPS and provide evidence for clinical practice.Method Randomized controlled trials(RCTs)of extracorporeal magnetic stimulation in the treatment of CP/CPPS(2000-29th April 2022)were retrieved from Pubmed,EMBASE,the Cochrane library,Web of Science,CNKI and Wanfang.The data were collected by two individual analyzer and analyzed by Revman 5.4.Results 5 RCTs with 364 patients with CP/CPPS were included for Meta-analysis.Compared with the control group,extracorporeal magnetic stimulation could improve NIH/CPSI[MD=5.61;95%CI(-7.91,-3.31),P<0.00001],and Qmax[MD=3.99;95%CI(2.63,5.35),P<0.00001]significantly.However,no significant difference was found in IIEF-5[MD=-0.94;95%CI(-2.08,0.21),P=0.11].Conclusion Extracorporeal magnetic stimulation significantly improves symptoms of CP/CPPS,NIH-CPSI score and Qmax.However,high-quality multicenter clinical trials with a large sample size are needed to verify the results.
