激光联合光动力疗法治疗女性外阴后联合尖锐湿疣对性功能的影响

摘　　要：目的:观察采用超脉冲CO_(2)激光联合5-氨基酮戊酸光动力疗法(ALA-PDT)治疗女性外阴后联合尖锐湿疣(CA)对患者性功能的影响。方法:干预组为63例女性外阴后联合CA患者,采用超脉冲CO_(2)激光联合ALA-PDT治疗,治疗间隔为7~10 d治疗1次,治疗12周。对照组:单纯采用超脉冲CO_(2)激光治疗女性外阴后联合CA患者63例,治疗间隔为7~10 d治疗1次,治疗12周。于治疗结束后12周筛选疣体完全清除者进行随访,评估治疗结束24周后对患者性功能的影响。结果:治疗结束12周后,干预组53例(84.13%)患者疣体完全清除;对照组21例(33.33%)患者疣体完全清除。治疗结束24周后,干预组治疗前、后女性性功能指数(FSFI)比较差异无统计学意义(P>0.05);与治疗前比较,对照组治疗后患者性高潮与性生活满意度下降,差异有统计学意义(P<0.05)。治疗结束半年后干预组FSFI评分[(28.38±1.67)分]高于对照组[(21.85±2.59)分],差异有统计学意义(P<0.05)。结论:超脉冲CO_(2)激光联合PDT治疗女性外阴后联合CA可减少对患者性功能的影响。Objective:To observe the effect of ultra-pulsed CO_(2)laser combined with 5-aminolevulinic acid photodynamic therapy(ALA-PDT)for female posterior vulvar union condyloma acuminatum(CA)on the sexual function.Methods:Sixtythree female patients with CA on the posterior vulvar union were selected as treatment group and were treated with ultrapulsed CO_(2)laser combined with ALA-PDT.The treatment interval was 7-10 days,for a course of 12 weeks.A total of 63female posterior vulvar union with CA were selected as control group and were treated with ultra-pulsed CO_(2)laser alone,and the treatment interval was 7-10 days.Twelve weeks after the treatment,the patients with complete wart removal were selected for follow-up,and the impact on the sexual function of the patients was evaluated 24 weeks after the treatment.Results:After12 weeks of treatment,53 patients(84.13%)in the treatment group were completely cleared of warts,and 21 patients(33.33%)in the control group were completely cleared of warts.Sexual function was assessed 24 weeks after treatment.There was no statistical significance in the female sexual function inventory(FSFI)before and after treatment in the treatment group(P>0.05),while the orgasm and sexual satisfaction of the control group decreased after treatment,with statistical difference(P<0.05).Six months after the treatment,the FSFI in the treatment group was 28.38±1.67 higher than that in the control group(21.85±2.59),with statistical difference(P<0.05).Conclusion:Combined PDT with ultra-pulsed CO_(2)laser for CA can reduce the impact on sexual function in female posterior vulvar union.
