新生幼驹先天性腹股沟阴囊疝的手术治疗

摘　　要：马腹股沟阴囊疝是新生公驹比较常发的外疝性疾病之一。其病因有先天与后天之分,先天性患驹是腹股沟环过大,幼驹常在出生后即发病。临床经诊6例病驹均为可复性、一侧性疝(其中左侧4例、右侧2例),有轻度疝痛症状,局部触压阴囊无热无痛,改变体位时疝内容物可还纳回腹腔,异常扩大的腹股沟外环(疝轮)可伸入3~5指。在根治手术中采用腹股沟外环切开法,即术部切开、还纳疝内容物、切除经结扎的鞘膜管(保留睾丸的病驹不做)、闭锁腹股沟外环(疝轮)等。因该病具有遗传性,所以其中5例公驹与去势术同时进行,只有1例按畜主要求保留了睾丸。术后加强对病驹的护理,经诊6例病例均治愈。Equine inguinal scrotal hernia is a common external hernia disease in newborn males.Its etiology can be congenital or acquired.Congenital inguinal scrotal hernia is the enlargement of the inguinal ring,which often occurs at birth in young foals.Clinical diagnosis showed that all 6 cases were reversible hernias,all of which were unilateral(including 4 on the left and 2 on the right).The foal had mild symptoms of hernias,and local contact with the scrotum was not hot or painful.When the body position was changed,the contents of the hernia could be returned to the abdominal cavity,and the abnormally enlarged external inguinal ring(hernia wheel)could extend into 3-5 fingers.In radical surgery,the external inguinal ring incision method was used,which included incision at the surgical site,removal of hernia contents,removal of ligated tunica vaginalis(without retaining the testicles),and closure of the external inguinal ring(hernia wheel).Due to the hereditary nature of the disease,5 of the male foals underwent castration simultaneously,and only 1 case retained the testicles according to the owner’s request.Adhering to the concept of sterility during surgery,cautious operation,using antibiotics to control infection,and strengthening postoperative care of the foal,all 6 cases were cured after diagnosis.
