摘　　要：目的:分析腹腔镜治疗的子宫肌瘤患者术前口服米非司酮时应用情境代入式护理的效果。方法:收集2019年8月—2022年8月本院诊断为子宫肌瘤患者76例为研究对象,随机分为观察组和对照组,每组38例。两组均予以米非司酮口服+腹腔镜肌瘤切除术,对照组予以常规护理,观察组则在此基础上予以情境代入式护理,比较两组术中出血量、术后血红蛋白水平、子宫体积、孕酮(P)、雌二醇(E_(2))、用药知识掌握情况、用药行为、术后月经恢复时间及术后48h内疼痛呼叫例次。结果:观察组术中出血量低于对照组、血红蛋白高于对照组,差异有统计学意义(P<0.05);观察组术后P、E2水平低于对照组(P<0.05);观察组用药知识得分、用药行为得分均高于对照组,且疼痛呼叫例次均少于对照组,月经恢复时间长于对照组,差异有统计学意义(P<0.05)。结论:子宫肌瘤患者腹腔镜手术前口服米非司酮时予以情境代入式护理是可行的,既能提高用药依从性及正确用药行为,还能有效控制术后性激素水平,加快术后康复进程。Objective:To analyze the effect of situational substitution nursing in patients with uterine fibroids treated by laparoscopy before oral administration of mifepristone.Methods:A total of 76 patients with uterine fibroids diagnosed in our hospital from August 2019 to August 2022 were randomly divided into observation group and control group,with 38 patients in each group.Both groups were given mifepristone orally+laparoscopic myomectomy.The control group was given routine nursing,while the observation group was given situational substitution nursing on this basis.The intraoperative blood loss,postoperative hemoglobin level,uterine volume,progesterone(P),estradiol(E_(2)),mastery of medication knowledge,medication behavior,postoperative menstrual recovery time and pain call cases within 48 hours after operation were compared between the two groups.Results:The intraoperative blood loss in the observation group was lower than that in the control group,and the hemoglobin was higher than that in the control group,the differences were statistically significant(P<0.05).The levels of P and E2 in the observation group were lower than those in the control group(P<0.05).The scores of medication knowledge and medication behavior in the observation group were higher than those in the control group,and the number of pain calls was less than that in the control group,and the menstrual recovery time was longer than that in the control group,the differences were statistically significant(P<0.05).Conclusion:It is feasible to give situational nursing to patients with uterine fibroids before laparoscopic surgery when taking mifepristone orally.It can not only improve medication compliance and correct medication behavior,but also effectively control postoperative sex hormone levels and accelerate postoperative rehabilitation process.
