磁疗联合本体感觉控制训练对老年膝骨性关节炎患者的影响

摘　　要：目的 探究磁疗联合本体感觉控制训练对老年膝骨性关节炎患者膝关节功能及预后的影响。方法 选取新疆医科大学第五附属医院2019年1月~2021年3月收治的120例老年膝骨性关节炎患者作为研究对象,按照随机数字表法分成两组,各60例,剔除无法配合研究者,最终109例完成研究,其中对照组58例,观察组51例。两组均给予常规康复治疗,对照组在常规康复治疗的基础上给予负荷控制下的本体感觉训练,观察组在对照组基础上给予脉冲热磁疗法治疗,两组均连续治疗6周。比较两组干预治疗前后Lysholm膝关节功能评分、视觉模拟疼痛评分(VAS)、稳定指数、躯体功能变化(30 s椅子坐起试验、40 m快走试验所需时间)以及随访1年的复发率。结果 治疗后观察组Lysholm膝关节功能评分高于对照组,VAS评分及稳定指数中总稳定指数、前/后稳定指数、左/右稳定指数均低于对照组,躯体功能指标中30 s椅子坐起试验次数多于对照组,40 m快走试验所需时间短于对照组,随访1年复发率低于对照组,差异均有统计学意义(P<0.05)。结论 磁疗联合本体感觉控制训练可有效促进老年膝骨性关节炎患者的膝关节功能改善,减轻疼痛程度、增加膝关节稳定性、提升躯体功能,短期不易复发,预后表现较好。Objective To investigate the effect of magnetic therapy combined with proprioceptive control training on knee joint function and prognosis in elderly patients with knee osteoarthritis.Methods A total of 120 elderly patients with knee osteoarthritis admitted to the Fifth Affiliated Hospital of Xinjiang Medical University from January 2019 to March 2021 were selected as the study subjects and divided into two groups according to the random number table,60 cases for each group.The researchers could not cooperate were excluded,and 109 cases finally completed the study,including 58 cases in the control group and 51 cases in the observation group.Both groups were given conventional rehabilitation,the control group was given proprioceptive training under load control on the basis of conventional rehabilitation,and the observation group was given pulsed thermomagnetic therapy on the basis of the control group,and both groups were continuously performed for 6 weeks.Lysholm knee function score,visual analogue scale(VAS),stability index,physical function changes(time required for 30 s chair sitting test,40 m brisk walking test),and recurrence rate at 1 year of follow-up were compared between the two groups before and after intervention.Results After treatment,the Lysholm knee function score of the observation group was higher than that of the control group,the VAS score and the total stability index,anterior/posterior stability index,and left/right stability index of the stability index were lower than those of the control group,the number of 30 s chair sitting test in the physical function index was more than that of the control group,the time required for the 40 m brisk walk test was shorter than that of the control group,and the recurrence rate at 1 year of follow-up was lower than that of the control group,and the differences were statistically significant(P<0.05).Conclusion Magnetic therapy combined with proprioceptive control training can effectively promote the improvement of knee joint function,reduce the degree of
