摘 要:目的:分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清主要肿瘤标记物[癌胚抗原(carcinoembryonic antigen,CEA)、甲胎蛋白(alpha-fetoprotein,AFP)等胚胎抗原类标记物和糖类抗原125(carbohydrate antigen 125,CA125)、糖类抗原19-9(carbohydrate antigen 19-9,CA19-9)、糖类抗原15-3(carbohydrate antigen 15-3,CA15-3)等糖蛋白类标记物]的表达水平,并探讨其异常升高的相关影响因素。方法:选择2022年1月至8月遂宁市中心医院内分泌代谢病科收治的202例T2DM患者作为观察组,随机选取同期在本院进行健康体检的200例健康体检者作为对照组,比较两组血清CEA、AFP、CA125、CA19-9、CA15-3水平,进一步将观察组的T2DM患者分为肿瘤标记物异常升高组和正常组,对比两亚组糖脂代谢及其他指标的差异,采用Logistic回归模型筛选影响T2DM患者肿瘤标记物异常升高的主要因素。结果:T2DM患者的血清CEA、CA125和CA19-9水平明显高于健康对照人群,差异有统计学意义(均P<0.05)。肿瘤标记物异常升高组T2DM患者的平均年龄、空腹血糖(fasting plasma glucose,FPG)、餐后2小时血糖(2-hour postprandial plasma glucose,2h PG)、糖化血红蛋白(glycosylated hemoglobin A1c,Hb A1c)、总胆固醇(total cholesterol,TC)、甘油三酯(triglycenide,TG)明显高于肿瘤标记物正常组,差异均有统计学意义(均P<0.05)。多因素Logistic回归分析,结果显示:年龄(OR=1.289)、FPG(OR=4.309)、2h PG(OR=1.732)、Hb A1c(OR=4.888)、TC(OR=2.667)、TG(OR=5.054)的高水平状态是T2DM患者血清标记物异常升高的独立危险因素(均P<0.05)。结论:T2DM患者血清CEA、CA125和CA19-9高于健康人群水平,T2DM患者部分肿瘤标记物异常升高可能主要与糖脂代谢异常有关。Objective:To analyze the expression levels of major tumor markers[embryonic antigen markers such as carcinoembronic antigen(CEA)and alpha-fetoprotein(AFP),and glycoprotein markers such as carbohydrate antige n 125(CA125),carbohydrate antigen19-9(CA19-9)and carbohydrate antigen 15-3(CA15-3)]in the serum of patients with type 2 diabetes mellitus(T2DM),and explore the influencing factors of abnormal increase of tumor markers.Methods:202 case of T2DM admitted to the endocrine and metabolic department in Suining Central Hospital from January 2022 to August 2022 were selected as the observation group,and 200 health examinees who had health checkups in the same hospital at the same time were randomly selected as the control group.The serum CEA,AFP,CA125,CA19-9 and CA15-3 levels in the two groups were compared.T2DM patients in the observation group were further divided into two groups:abnormally high tumor marker group and normal tumor marker group.The differences in glycolipid metabolism and other indicators between the two subgroups were compared.The logistic regression model was used to screen the main factors affecting the abnormal increase of tumor markers in T2DM patients.Results:The serum CEA,CA125 and CA19-9 levels in T2DM patients were significantly higher than those in healthy controls(P<0.05).The mean age,fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2h PG),glycosylated hemoglobin A_(1C)(HbA_(1C)),total cholesterol(TC)and triglyceride(TG)of T2DM patients in the abnormally high tumor marker group were significantly higher than those in the normal tumor marker group(P<0.05).Multivariate logistic regression analysis showed that high levels of age(OR=1.289),FPG(OR=4.309),2h PG(OR=1.732),HbA_(1C)(OR=4.888),TC(OR=2.667)and TG(OR=5.054)were independent risk factors for abnormal elevation of serum markers in T2DM patients(all P<0.05).Conclusion:The serum CEA,C A125 and C A19-9 levels in T2DM patients were higher than those in healthy people.The abnormal increase of some tumor markers in T2DM patients
