免充气经腋窝入路腔镜下单侧甲状腺切除术的疗效分析

摘　　要：目的探讨免充气经腋窝入路腔镜下单侧甲状腺切除术的临床效果。方法2021年7月至2022年8月纳入40例行单侧甲状腺切除患者的临床资料进行回顾性分析,依其手术方式不同分为观察组与对照组,每组20例。观察组采用免充气经腋窝入路腔镜下单侧甲状腺切除术,对照组采用传统经颈前入路单侧甲状腺切除术。比较两组的手术时间、术中出血量、术后第1天引流量、手术并发症、切口疼痛情况及患者对切口美观满意度。结果观察组手术时间长于对照组,术后第1天引流量多于对照组,组间比较差异有统计学意义(P<0.001);两组在术后切口疼痛、术后住院时间、总并发症方面差异无统计学意义(P>0.05)。而在切口美观满意度方面,观察组明显高于对照组(P<0.001)。结论免充气经腋窝入路腔镜下单侧甲状腺切除术是安全、有效的术式,同时具有良好的美容效果。Objective To study the clinical effect of endoscopic unilateral thyroidectomy through axillary approach without aeration.Methods The clinical data of 40 patients undergoing unilateral thyroidectomy from July 2021 to August 2022 were retrospectively analyzed.They were divided into observation group and control group,with 20 cases in each group.The observation group underwent endoscopic unilateral thyroidectomy through axillary approach without aeration.The control group received unilateral thyroidectomy through traditional anterior cervical approach.And then,operation time,intraoperative blood loss,the volume of drainage on the first day after surgery,surgical complications,incision pain and patients'satisfaction with incision aesthetics were compared between the two groups.Results The operation time of the observation group was longer than that of the control group,and the drainage volume on the first day after surgery was higher than that of the control group,and difference between groups was statistically significant(P<0.001).There was no statistically significant difference between the two groups in terms of postoperative incision pain,postoperative hospital stay,and total complications(P>0.05).Satisfaction with incision aesthetics in the observation group was significantly higher than that in the control group(P<0.001).Conclusion Endoscopic unilateral thyroidectomy through axillary approach without aeration is safe and effective,and it has good cosmetic effect on incision,
