不同入路方式下双钢板固定治疗SchatzkerⅤ型及Ⅵ型胫骨平台骨折的临床观察

摘　　要：目的探讨不同入路方式下双钢板固定治疗SchatzkerⅤ型、Ⅵ型胫骨平台骨折的效果。方法抽取2021年1月至2022年12月郑州大学第一附属医院收治的胫骨平台骨折患者80例,均为SchatzkerⅤ型、Ⅵ型,依照手术方式将其分为对照组和研究组,每组40例。对照组单纯予以前外侧入路双钢板固定治疗,研究组予以前后联合入路双钢板固定治疗。比较两组手术情况、术后骨折复位、术后膝关节功能恢复及并发症发生情况。结果两组手术时间及术中出血量比较差异未见统计学意义(P>0.05);研究组术后愈合时间、患肢负重时间少于对照组(t=2.45、6.31,P均<0.05)。研究组术后骨折复位情况、膝关节功能恢复程度优于对照组(χ2=6.28、9.80,P均<0.05)。研究组并发症发生率(2.50%,1/40)低于对照组(25.00%,10/40),P<0.05。结论前后联合入路双钢板固定可缩短胫骨平台骨折患者的预后恢复时间,提高负重能力,确保术后骨折复位以及膝关节功能,并发症发生率低。ObjectiveTo investigate the effects of double-plate fixation via different approaches in the treatment of Schatzker type V and type VI tibial plateau fractures.MethodsA total of 80 patients with tibial plateau fractures admitted to the First Affiliated Hospital of Zhengzhou University from January 2021 to December 2022 were selected,and all of them were Schatzker type V or type VI.The selected patients were divided into control group and research group according to the surgical method,with 40 cases in each group.The control group was treated by double-plate fixation via anterolateral approach,and the research group was treated by double-plate fixation via anterior-posterior approach.The surgical conditions,postoperative fracture reduction,postoperative knee function recovery and complications of the two groups were compared.ResultsThere was no significant difference in operation time and intraoperative blood loss between the two groups(P>0.05).The postoperative healing time and weight-bearing time of the affected limb in the research group were lower than those in the control group(t=2.45,6.31;all P<0.05).The postoperative fracture reduction and the degree of knee function recovery in the research group were better than those in the control group(χ2=6.28,9.80;all P<0.05).The complication rate in the research group(2.50%,1/40)was lower than that in the control group(25.00%,10/40),P<0.05.ConclusionsDouble-plate fixation via anterior-posterior approach can shorten the recovery time of patients with tibial plateau fractures,improve weight-bearing capacity,ensure postoperative fracture reduction and knee joint function,with low complication rate.
