摘　　要：目的探讨Bankart合并Hill-Sachs损伤采取关节镜下Bankart修补并Remplissage术治疗的效果。方法抽取解放军联勤保障部队第九八八医院2018年12月至2021年12月收治的Bankart合并Hill-Sachs损伤患者35例作为研究对象,均接受关节镜下Bankart修补并Remplissage术治疗。所有患者临床资料完整,随访≥6个月,总结临床效果。结果所有患者均顺利完成手术,手术期间无严重并发症,随访半年有3例患者主诉天气变化或下雨有关节酸痛但可忍受,不影响正常生活或工作;术后疼痛评分低于术前(P<0.05);术后半年美国肩肘外科协会评分,屈肘90°外旋、前屈上举、外展上举程度均高于术前,西安大略肩关节不稳指数评分低于术前,差异有统计学意义(P<0.05)。结论Bankart合并Hill-Sachs损伤采取关节镜下Bankart修补并Remplissage术治疗,手术成功率高,并发症少,且可改善术后患者肩关节稳定性,降低患者肩关节的再脱位发生率。ObjectiveTo investigate the therapeutic effect of arthroscopic Bankart repair combined with Remplissage surgery on Bankart lesion complicated by Hill-Sachs defect.MethodsThirty-five patients with Bankart lesion complicated by Hill-Sachs defect admitted to No.988 Hospital of the Joint Logistic Support Force from December 2018 to December 2021 were selected as the study subjects,and they were all treated by arthroscopic Bankart repair combined with Remplissage surgery.The clinical data of them were complete,and they were followed up for 6 months or more.The clinical effects were summarized.ResultsAll patients successfully underwent the operation completely,without serious complications during the operation.During the 6-month follow-up,3 patients complained of tolerable joint sour pain when weather changed or rained,which did not affect their normal life or work;the postoperative pain score was lower than preoperative score(P<0.05).Half a year after surgery,American Shoulder and Elbow Surgeons’Form score and range of motion(external rotation of 90°elbow flexion,upward lifting of elbow forward flexion,upward lifting abductions)were higher,while Western Ontario shoulder instability index score was lower,compared with indexes before surgery(P<0.05).ConclusionsArthroscopic Bankart repair combined with Remplissage surgery,in the treatment of Bankart lesion complicated by Hill-Sachs defect,has a high success rate and fewer complications,moreover,it can improve postoperative shoulder stability and reduce the re-dislocation rate of shoulder joint.
